Your day to day as a Care Manager

As a Care Manager at MonmouthCares, you will play a crucial role in meeting with families to assess their needs and coordinate care for children and youth facing mental health, behavioral health, and intellectual/developmental challenges. Requirements:

Knowledge and skills required for the position are:

  • Experience as a Social Worker from social services or Human Services
  • Minimum Bachelor's degree in related field
  • Detail oriented and deadline aware
  • Responsible and accountable
  • Knowledge of educational system including special education
  • Child welfare system
  • Juvenile justice system and child protection services
  • Minimum 1-year experience working with at-risk children and their families
  • Bi-lingual ability in Spanish, Portuguese or Creole highly desirable
