Description:

The mission of the Tunnel to Towers Foundation is to honor the sacrifice of firefighter Stephen Siller who laid down his life to save others on September 11, 2001. We also honor our military and first responders who continue to make the supreme sacrifice of life and limb for our country.

The Tunnel to Towers Foundation is committed to eradicating homelessness amongst our veterans. The Foundation is seeking an experienced Social Services Case Manager for our homeless veteran program to support the Social Service Manager, and requests for support as part of the Homeless Veteran Program.

The Foundation is seeking a licensed social worker to be responsible for managing a case-load of individual recipients who become part of the Veteran Homeless Program. As case manager the successful candidate will be responsible for the communication between the Foundation, recipient of assistance and family members as they negotiate the intake and acceptance process. The case manager will address any immediate psychosocial needs to determine if referral for local support or additional resources are indicated.

Applicants must be well organized, able to effectively multi-task within a timely fashion, possess a keen attention to detail, able to prioritize workload and handle sensitive information.

Requirements:
  • Responsible for communication between the Foundation, recipient and family members throughout the acceptance process and any continued follow-up.
  • Prepare all necessary documentation for possible recipient.
  • Maintain current files on program recipient’s and document all communication with recipient’s.
  • Psychosocial advocacy when indicated, provide compassionate support to spouses who may be in need of short term or ongoing counseling by finding appropriate resources for family.
  • Ensure that ongoing psychosocial needs are being addressed through follow up communication with the families.
  • Administrative tasks that will include email and phone correspondence, maintaining files and spreadsheets, and conducting research for appropriate resources for recipients.

Qualifications

  • Must be willing to travel within the United States on behalf of the Foundation as needed.
  • Ability to handle sensitive information and confidential matters with discretion.
  • Ability to work independently in fast paced environment and multi-task in a smart and timely fashion.
  • Must be detailed oriented.
  • Knowledge of health care and social service system entitlements
  • Must possess the ability to communicate effectively and appropriately in writing and verbally.
  • Must have the quick thinking and problem-solving skills to effectively address issues that may occur.
  • Must have the organizational skills to multi-task and handle various duties in short timeframes.
  • Must have 3 year’s social worker experience
  • Master’s degree in Social Work accredited by CSWE, (LMSW/LCSW) preferred
  • Research and locate resources & social services for qualifying Veterans outside of the established network of coverage
  • Veterans encouraged to apply
  • Pre-employment screening, including a background check, is required.

Working conditions

Special Working conditions may include: a change in days worked, shift hours, and/or working conditions (outdoors or other locations) when participating and assisting in special events such as: NYC Tunnel to Towers Run, golf outings, and other fundraising events, as required.
