SCHEDULE
Full-time, Monday-Friday during business hours, including sharing in the crisis rotation as scheduled

CPLC CARF accredited Centro Esperanza offers specialized services for SMI populations focusing on case management and psychiatric services. We provide trauma-informed support tailored to individual needs supporting the people we serve recovery journey.

HOW WILL YOU MAKE AN IMPACT?
As the Clinical Coordinator, you will be responsible for leadership and clinical oversight, training and mentoring for Case Managers and Specialist staff on assigned team. You will supervise clinical activities ensuring the program services are provided in accordance with policies and procedures, local, state, and federal laws, regulation, and licensure/accreditation standards. You will ensure assessments and individual service plans are developed and implemented in accordance with ADHS/DBHS guidelines for each client on the team. You will get to create a climate which promotes quality treatment for each client. You will ensure productivity goal expectations meet or exceeded by direct reports, supervising clinical staff as assigned, participating in staff performance and evaluation activities, coordinating/facilitating training and clinical supervision as assigned. Working as a member of the treatment team, you will get to provide direct services as assigned, serve as community liaison and Centro Esperanza representative as assigned.

SUPERVISORY RESPONSIBILITIES
You will direct the day-to-day clinical and administrative oversight of assigned Case Managers and Specialty staff at the Centro Esperanza SMI outpatient behavioral health program. Sharing rotation of on-call after-hours crisis cell phone as needed is part of the Clinical Coordinator's role.

PRIMARY DUTIES AND ESSENTIAL FUNCTIONS

  • Provide and document clinical and administrative supervision, consultation, and mentoring of daily operations for direct reports.
  • Ensure that the needs of clients are met and that services are delivered as outlined in each client’s assessment and Individual Service Plan.
  • Provide clinical oversight of services provided, assessments conducted, and Individual Service Plans developed for all clients assigned to the team.
  • Assist with both Substance Abuse and Housing needs for the team.
  • Monitors and evaluates Case Managers and Specialist staff overall performance.
  • Monitor productivity of assigned staff ensuring that expectations are met.
  • Ensure that direct reports provide coordination of care to internal and external provider agencies, state entities or other stakeholders (e.g. probation officers, advocates, family member, hospital staff, jail staff, vocational rehabilitation, etc.) as needed.
  • Prepare reports as required by Clinical Director or Site Administrator. Reporting includes, but is not limited to the following: Appendix C standards, clients in residential placement, clients living in Supervisory Care Homes, AHCCCS applications submitted, internal and external corrective action plans (CAPs) etc.
  • Monitor population on Court Ordered Treatment (civil and criminal) to ensure that consumer and team adhere to all terms of the court order. Prepare monthly COT report and submit to Clinical Director.
  • Ensure that staff communicate with courts as required either in person or written report and that such communication is consistent with the assessments, Individual Service Plans, and team recommendations for the client.
  • Work with the clinical team to ensure clients receive appropriate care within the least restrictive setting, including coaching and mentoring specific to inpatient and residential treatment discharge plans.
  • Establish team workflows and standards; approve complex service plans; adjust caseload assignments; issue corrective actions; make hiring recommendations that carry particular weight.
  • Bring to the attention of the Clinical Director and Site Administrator any unmet service needs identified by line staff in the treatment planning process and assist line staff in identifying interim services available to meet client needs.
  • Share rotation of on-call crisis cell phone as needed. After-hours duties are escalation/triage and decision-making, not routine case processing.

MINIMUM QUALIFICATIONS AND COMPETENCIES

  • Bachelor’s degree Social Work, Psychology or related field.
  • At least two (2) years of behavioral health experience.
  • Management experience preferred.
  • Generalist knowledge in general mental health, substance abuse, co-occurring disorders, and serious mental illness.
  • Possess a valid driver’s license and ability to be insured under automobile insurance policy.
  • Able to work a flexible schedule as needed per program requirements.
  • Ability to obtain fingerprint clearance card, CPR and First Aid.
