Position Summary
SmithLife Homecare is seeking an experienced and compassionate Licensed Practical Nurse (LPN) to join our team on a PRN (as-needed) basis. In this flexible role, you will provide skilled nursing services to clients in their homes, supporting safe, high-quality care and promoting independence.
Working under the supervision of a Registered Nurse (RN), you will implement individualized care plans, monitor client conditions, and support families throughout the care journey.

Key Responsibilities
- Deliver skilled nursing care in accordance with the established Plan of Care
- Administer medications, injections, and treatments as prescribed
- Monitor and document client condition, including vital signs and response to care
- Observe and report changes in physical, emotional, or cognitive status
- Assist with clinical procedures including wound care and infection prevention
- Support clients with activities of daily living as needed
- Educate clients and families on care needs and disease management
- Maintain accurate, timely documentation in accordance with agency standards
- Collaborate with RNs, care coordinators, and interdisciplinary team members
- Follow all infection control protocols and agency policies
Qualifications
- Active LPN license in Maryland and/or Washington, DC (in good standing)
- Graduate of an accredited practical nursing program
- Minimum 3 years of clinical nursing experience preferred
- Strong clinical judgment, communication, and documentation skills
- Experience in home care, geriatrics, or chronic disease management preferred
- Ability to work independently and adapt in a home-based setting
Physical Requirements
- Ability to lift, assist, and reposition clients as needed (50+ lbs)
- Frequent standing, walking, bending, and kneeling
- Full use of hands and arms for clinical tasks
Schedule & Status
- PRN / Per Diem (As Needed)
- Variable hours based on client needs, including potential evenings, weekends, or short-term assignments
Compensation
- Competitive hourly rate based on experience
- Opportunities for additional hours based on performance and availability
