Position Overview We’re hiring Licensed Practical Nurses (LPNs) to provide skilled nursing care in patients’ homes. You’ll play a vital role in helping patients maintain independence while delivering high-quality, compassionate care where it matters most—at home.

Responsibilities Perform in-home nursing assessments

Provide skilled nursing care including wound care, medication administration, injections, catheter care, and vital signs

Monitor patient conditions and report changes to supervising RN/provider

Educate patients and caregivers on medications and self-care

Collaborate with interdisciplinary care teams

Maintain accurate EHR documentation

Follow infection control and safety protocols

Qualifications Graduate of an accredited LPN/LVN program

Active LPN license in Maryland

2–3 years of nursing experience (home health preferred)

Strong clinical and communication skills

Experience with electronic health records

Valid driver’s license and reliable transportation

Willingness to travel within Prince George’s & Montgomery counties
