Job Summary
The Clinical Manager, reporting to the Administrative Director, is responsible for ensuring high-quality, compliant care in alignment with the organization's mission, vision, policies, and regulatory standards. This role provides clinical leadership through planning, coordination, implementation, and evaluation of home health services, while supervising and supporting field staff. The Clinical Manager collaborates closely with the Administrator(s), interdisciplinary team, patients and families, and referring providers and agencies to ensure effective, patient-centered care delivery.
Supervises the Following Positions
Positions: Home Health Case Managers, RNs, LVNs, Aides
Physical Requirements

Performance: Position Specific Essential Functions
- Plan, coordinate, manage, and evaluate the activities of assigned clinical teams to ensure high-quality home health services that meet organizational standards and comply with federal, state, and local regulations.
- Supervise, coach, and support assigned field staff to promote efficient, cost-effective, and quality-driven care.
- Oversee the implementation of patient care plans, including recertifications, discharge planning, and continuity of care.
- Coordinate and lead regularly scheduled multidisciplinary case conferences and team meetings.
- Ensure timely, accurate, and compliant clinical documentation of field staff.
- Monitor staff productivity, performance, and time management, providing ongoing feedback and corrective action as needed.
- Assess staffing patterns and workload using performance indicators to maintain appropriate staffing levels.
- Supervise patient visit scheduling to ensure timeliness and quality of service.
- Serve as a clinical and regulatory resource for staff regarding policies, procedures, regulations, and coverage issues.
- Maintain open, honest, and effective communication with all levels of staff and management.
- Direct clinical staff to appropriate internal and community resources to support patient care.
- Conduct in-home supervisory visits as needed and in preparation for annual performance reviews.
- Complete annual performance reviews in a timely, constructive manner.
- Assist with staff orientation, education, and ongoing development programs in collaboration with the QA RN and management.
- Collaborate with the management team on organization-wide initiative, including participation in the Performance Improvement Plan and any assigned committees and/or initiatives.
- Manage physician orders, documentation, and workflows within the EMR.
- Partner with intake to support timely admissions and reduce referral delays.
- Review clinical reports (i.e. occurrences, infection control, on call) and take appropriate action when needed.
- Ensure compliance with clinical, state, and federal regulations.
- Assist with budgetary process and financial operations as needed.
- Performs other duties as assigned.
Qualifications
Education
- Required
- High School Diploma, GED, or equivalent
- Graduate of Approved Program of Nursing
- Associate’s or Bachelor’s degree in Nursing
Experience:
- Required
- One year of experience as a Registered Nurse
- Preferred
- 1-2 years of supervisory experience in Home Health
Certification/Licensure:
- Required
- Registered Nurse (RN), with authorization to practice in the State of Texas
- Basic Life Support (BLS) Certification
- Must obtain within ninety (90) days of start date
