Position OverviewQueens Community House is seeking a highly skilled Senior Health Monitor to join our Queens Childcare Network team. In this role, you will be responsible for monitoring and assessing client health, ensuring high-quality care, and supporting the development and implementation of individualized care plans.
The Senior Health Monitor plays a critical role in improving client outcomes and enhancing the overall well-being of children and families served across our network.
Key ResponsibilitiesConduct comprehensive and ongoing health assessments of clients
Document findings accurately and maintain up-to-date health records
Collaborate with healthcare professionals to develop and implement care plans
Monitor and report changes in client health status in a timely manner
Provide education, guidance, and support to clients and their families
Ensure compliance with all health, safety, and regulatory requirements
Bachelor’s degree in Nursing or a related field
Minimum of 5 years of experience in a healthcare setting
Strong assessment and critical thinking skills
Excellent communication and interpersonal skills
Master’s degree in Nursing or a related field
Relevant professional certification (e.g., RN, geriatric care certification)
Experience in home health, community health, or social service settings
Clinical assessment and care planning
Critical thinking and problem-solving
Effective communication and relationship-building
Cultural competency and client-centered approach
Strong documentation and organizational skills
