Primary Care APP- Ellis Primary Care at Nott Street & State Street Mental Health- Schenectady, NY in Schenectady, New York at Ellis Medicine

Ellis Medicine is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join our integrated primary care team serving both Ellis Primary Care at Nott Street and State Street Mental Health Primary Care in Schenectady, New York.

This unique opportunity offers the ability to provide comprehensive primary care across two outpatient settings while supporting Ellis Medicine’s commitment to integrated, whole-person care. The provider will care for patients across the lifespan in a traditional primary care setting while also providing medical care to individuals receiving behavioral health services. This collaborative model promotes access to primary care, improves management of chronic medical conditions, and supports coordination between medical and behavioral health providers.

Position Highlights

• Full-time outpatient position
• Split between two Ellis Medicine practices:
  • 0.40 FTE (16 hours/week) at Ellis Primary Care at Nott Street
  • 0.60 FTE (24 hours/week) at State Street Mental Health Primary Care
• Comprehensive family medicine services in an established outpatient setting
• Integrated primary care services for individuals receiving behavioral health treatment

Clinical Responsibilities

The Family Medicine NP or PA will provide comprehensive outpatient primary care services across both practice locations, including:

• Delivering preventive, acute, and chronic medical care
• Performing comprehensive health assessments and physical examinations
• Providing annual wellness visits and age-appropriate preventive care
• Diagnosing and treating acute illnesses and injuries
• Managing chronic medical conditions and developing individualized treatment plans
• Providing pediatric, adolescent, adult, and geriatric primary care at Ellis Primary Care at Nott Street
• Providing routine medical care to patients receiving behavioral health services at State Street Mental Health Primary Care
• Providing immunizations and recommended health screenings
• Providing health education, wellness counseling, and lifestyle modification guidance
• Monitoring medical conditions and medications in collaboration with behavioral health providers when appropriate
• Coordinating care with physicians, behavioral health professionals, specialists, and other members of the interdisciplinary team
• Supporting improved access to primary care for patients with complex medical and behavioral health needs

Qualifications

• Current NP or PA certification
• Licensed (or eligible) to practice in New York State
• Experience in family medicine, primary care, or an outpatient setting required
• Experience caring for patients with behavioral health needs is preferred but not required
• Ability to work collaboratively within a multidisciplinary team
• Strong communication, clinical, and interpersonal skills
• Commitment to collaborative, patient-centered care

• Competitive Salary of $136,842 with the ability to earn additional compensation through productivity
• The posted salary reflects the starting range of total compensation and does not include productivity bonuses incentives related to quality and performance, extra shift incentives, and other forms of cash compensation as applicable to the position. Please note that this salary range is provided in accordance with State law and is subject to variation due to the factors noted above.
