Care Manager Team Leader

About this opportunity

As the Care Manager Team Leader, you will support and supervise Care Managers to deliver high-quality, relationship-centred community-based services. Reporting to the Managing Director, you will help ensure care management practice is consistent, compliant and aligned with legislative, regulatory and organisational requirements.

Key Responsibilities

  • Support consistent delivery of high-quality, relationship-centred services across the client journey.
  • Provide day-to-day supervision and guidance to Care Managers across care planning, documentation, risk monitoring and service coordination.
  • Act as the first point of escalation for complex client matters, safeguarding concerns and service risks.
  • Conduct regular audits of care plans, client reviews, budgets, risk assessments and documentation to maintain compliance and audit readiness.
  • Lead capability development through onboarding, mentoring, performance support and targeted education for Care Managers.

About you

  • A tertiary qualification in Nursing, Allied Health, Social Work or a related discipline will be highly regarded.
  • Demonstrated experience leading and developing high-performing teams within aged care, community services or a similar environment.
  • Confident working across quality, compliance, risk mitigation, documentation and continuous improvement requirements.
  • Strong analysis, problem-solving, organisational, time management and accurate data entry skills.
  • Ethical, self-motivated and collaborative, with a values-led, person-centred approach to service delivery.
