About the Role

We are seeking an experienced Registered Nurse (RN) to join our team as a Nurse Coordinator, working one day per week every Friday.

Based in Adelaide CBD, this role supports a veteran-focused patient cohort through a combination of clinical intake, telehealth screening, and nursing consultations, with flexibility to work from home when clinically appropriate.

You will work closely with the General Practitioner (GP) and multidisciplinary team to coordinate care, complete structured patient assessments, and support ongoing alternative healthcare pathways for patients with a range of healthcare needs.

This role is best suited to a nurse with strong experience in chronic pain management or mental health, who is confident working autonomously within a structured, private clinic environment.

Central to patient flow, clinical assessment, and continuity of care, you will ensure patients are appropriately screened, supported, and guided through their care journey, while maintaining high standards of clinical documentation and clear communication across the team.

Key Responsibilities

• Coordinate ongoing patient care across the General Practitioner (GP), allied health providers, and external services.
• Conduct structured patient intake assessments, clinical screening, and medical history reviews
• Support chronic pain management and care coordination
• Identify clinical risks and escalate concerns to the GP as required
• Prepare accurate, GP-ready clinical summaries to support treatment planning
• Deliver nursing consultations for new and existing patients within the clinic
• Maintain continuity of care through recalls, follow-ups, and ongoing patient engagement
• Build strong therapeutic relationships with veterans and their families
• Provide patient education, advocacy, and support across treatment pathways
• Identify psychosocial and mental health needs and coordinate appropriate referrals
• Maintain precise clinical documentation in line with AHPRA, Medicare, DVA, and privacy requirements
• Contribute to quality improvement and refinement of clinical processes

• Current unrestricted AHPRA registration as a Registered Nurse
• Minimum 2+ years post-registration clinical experience
• Strong background in chronic pain management or mental health
• Experience in patient pre-assessments and clinical intake screening
• Confident working independently in a care coordination role
• Advanced clinical assessment, triage, and documentation skills
• Strong communication skills with the ability to build rapport in complex care settings
• Highly organised with strong time management and attention to detail
• Experience with clinical software
• Patient-centred approach with a focus on continuity, quality, and outcomes

• Experience working with veterans or DVA patients
• Knowledge of Medicare and DVA processes
• Exposure to alternative healthcare approaches
