RN Field Supervisor - Per Diem - Rochester at Liberty Home Care – Niagara Falls, New York

Liberty Home Care is looking for a motivated Registered Nurse to join our growing home care nursing team on a Per Diem basis!

Schedule: 2 days per week 8:30am-4:30pm

Duties & ResponsibilitiesProvide assessment of newly referred patients and assess health needs in home care setting.Work with patient to create a patient centered plan of care.Reassess patient needs for services on a continual basis and discuss any changes in the plan of treatment with the patient physician/family.Prepare written documentation and summaries of the patient’s progress as needed for the continuation of home health services.Provide supervision and training of PCA/HHA staff.QualificationsNYS Licensed Registered Professional Nurse.Minimum of Associate's Degree required.One year Nursing experience required.Home care experience a plus.If you have an empathetic approach to helping others and thrive in a fast-paced environment, we invite you to apply.
