About This Position

Liberty Home Care is looking for a motivated Registered Nurse to join our growing home care nursing team on a Per Diem basis!

Schedule: 2 days per week 8:30am-4:30pm

Duties & Responsibilities
  • Provide assessment of newly referred patients and assess health needs in home care setting.
  • Work with patient to create a patient centered plan of care.
  • Reassess patient needs for services on a continual basis and discuss any changes in the plan of treatment with the patient physician/family.
  • Prepare written documentation and summaries of the patient’s progress as needed for the continuation of home health services.
  • Provide supervision and training of PCA/HHA staff.
Qualifications
  • NYS Licensed Registered Professional Nurse.
  • Minimum of Associate's Degree required.
  • One year Nursing experience required.
  • Home care experience a plus.
