Full-Time | Nashua, NH

Greater Nashua Mental Health is seeking a Case Manager to support individuals in Substance Use Disorder (SUD) treatment. This role provides outreach, engagement, and comprehensive case management to adults and adolescents, helping them access services, maintain treatment, and work toward recovery goals. The Case Manager collaborates closely with multidisciplinary teams and community partners to ensure coordinated, high-quality care.

Key Responsibilities

  • Provide outreach and engagement to individuals in SUD treatment, including community-based settings.
  • Collaborate with clients and clinical teams to assess ancillary service and resource needs and develop individualized case management plans to help support treatment goals.
  • Coordinate care with internal teams and external providers including medical, social service, and vocational resources.
  • Facilitate referrals, admissions, and discharges across levels of care.
  • Co-facilitate group treatment using evidence-based curricula.
  • Support treatment engagement through harm reduction education and program orientation.
  • Conduct proactive outreach to underserved populations to increase access to services.
  • Coordinate and support drug testing and communicate results appropriately.
  • Maintain accurate and timely documentation in compliance with regulatory requirements.
  • Communicate effectively with team members, providers, and stakeholders regarding client care.
  • Manage administrative responsibilities including email and scheduling in a timely and professional manner.
  • Perform additional duties or special projects as assigned.

Qualifications

· Bachelor’s degree in Psychology, Social Work, Human Services, or related field required.

· 1–2 years of experience in behavioral health, case management, or related field.

· Certified Recovery Support Worker (CRSW) preferred or willingness to obtain certification.

· Experience working with individuals with substance use disorders preferred.

· Familiarity with community resources and service systems.

· Strong communication, engagement, and interpersonal skills.

· Ability to work independently and collaboratively in a team environment.

· Strong organizational, time management, and multitasking skills.

· Proficiency with Microsoft Office applications.

· Ability to exercise sound judgment and maintain professional boundaries.
