Location: Developmental Disabilities - Lufkin, TX
Position Number: 1020
Salary: $42,120 - $43,555 annually, depending on experience and qualifications

About the Role
Burke is seeking a dedicated Habilitation Coordinator to support individuals with Intellectual and Developmental Disabilities (IDD) residing in nursing facilities. This position is responsible for helping individuals access specialized services that promote independence, community participation, and an enhanced quality of life.

The Habilitation Coordinator works closely with individuals, legally authorized representatives (LARs), nursing facility staff, and Service Planning Teams to coordinate services, conduct ongoing discovery activities, and update Habilitation Service Plans as needs evolve. Reliable transportation is required, as regular travel within the service area is expected.

What You'll Do
- Assist individuals residing in nursing facilities with accessing appropriate specialized services
- Coordinate services that support quality of life and community participation goals
- Conduct ongoing habilitation coordination and person-centered planning activities
- Participate in the discovery process to identify strengths, preferences, and support needs
- Develop and revise Habilitation Service Plans with the Service Planning Team
- Monitor progress and adjust service plans as assessments become available or needs change
- Collaborate with individuals, families, LARs, nursing facility staff, and community providers
- Maintain accurate documentation and comply with program requirements

What We're Looking For
- Bachelor's degree from an accredited college or university in Social Work, Rehabilitation, Psychology, Medicine, Nursing, Counseling, Sociology, Human Development, Gerontology, Educational Psychology, Criminal Justice, or with a major in a social, behavioral, or human services field
- One (1) year experience working or volunteering with individuals with IDD; or experience as a friend or family member of an individual with IDD
- Strong communication and interpersonal skills
- Ability to coordinate services and work collaboratively with multiple stakeholders
- Valid Texas driver's license

Work Schedule
- Monday-Friday
- 8:00 AM - 5:00 PM
- After-hours work may be required to meet consumer and program needs

Licenses & Certifications
- Valid Texas driver's license required
- Must be physically able to complete SAMA, CPR, First Aid, Defensive Driving, and all other required training
- Driving record must be insurable by Burke's insurance administrator
- Reliable transportation required

Physical Requirements
- Visual and auditory skills sufficient to evaluate and monitor consumer needs
- Fine motor skills for legible writing and typing
- Ability to bend, reach, and lift up to 25 pounds while assisting consumers and carrying charts or equipment
- Mobility required throughout the workday to support consumer services
