Halcyon Home is hiring a Home Care Care Coordinator with advancement potential. We are looking for a team player with strong computer skills and an eye for detail. Home Care and Personal Assistant Service scheduling experience are mandatory, along with a proven ability to manage a fast-paced, independent work schedule. Essential Functions:
  • Schedules and appropriately documents client schedules/changes/incidents in a timely manner.
  • Participates in coordinating care with management and client care team.
  • Supervises and manages assigned care team, including annual onsite visit and employee evaluations of caregivers.
  • Completes Annual Client Supervisory Visits.
  • Responsible for client file audits and rectifying discrepancies in a timely manner.
  • Provides effective communication to clients and families, community liaisons, other staff members and other health care professionals.
  • Responds to Agency and client needs in a professional manner.
  • Demonstrates commitment and competency.
  • Enhances professional growth and development through participation in educational programs, staff inservices and workshops
  • Promotes the Agency philosophy and administrative policies to ensure quality of care.

Education:

  • High School graduate or equivalent.

Licensure:

  • Current Texas driver’s license.

Experience:

  • One year of customer service experience preferred.
  • Home Care or Personal Assistant Service scheduling experience mandatory.

Skills:

  • Excellent computer skills.
  • Exceptional interpersonal, organizational, and communication skills.
  • Knowledge of medical terminology preferred.
  • Excellent phone skills.
  • Ability to travel locally with some exposure to inclement weather.

Transportation:

  • Reliable transportation and valid, current auto liability insurance.
Environmental and Working Conditions:
  • Routine office environment; noise level may be moderately high.
  • Must be able to work a flexible schedule and extended hours.
  • Participates in the On Call and Back Up On Call Care Coordinators schedule.
  • Possible exposure to toxic materials, toner, etc.
Physical and Mental Effort:
  • Prolonged standing and walking required.
  • Ability to lift up to 50 lbs.
  • Requires working under some stressful conditions to meet deadlines and client needs.
  • Ability to make quick decisions.
  • Excellent problem-solving skills and ability to self-manage.
