POSITION SUMMARY:

The Residential Services Coordinator serves as the primary point of contact and advocate for residents of Our Lady’s Sanctuary of Grace and Olathe Haven. This role coordinates individualized care plans, connects residents with essential community resources, provides ongoing emotional and practical support, and ensures each resident’s goals are progressing in a safe and structured environment. The Residential Services Coordinator embodies the mission and values of Catholic Charities of Northeast Kansas in all resident interactions. ESSENTIAL FUNCTIONS:

Intake & Assessment

  • Conduct comprehensive intake assessments for new residents, including psychosocial history, housing needs, prenatal care status, and immediate safety concerns.
  • Collaborate with the Senior Director of Housing to determine resident eligibility and appropriate level of care.
  • Orient incoming residents to house rules, expectations, program structure, and available services.
  • Coordinate move in and inventory of client’s property.

Case Management & Care Planning

  • Develop, implement, and regularly review individualized service plans (ISPs) in partnership with each resident.
  • Set measurable, time-bound goals related to prenatal health, parenting skills, education, employment, life skills, and housing stability.
  • Maintain accurate and confidential case files, progress notes, and documentation in compliance with organizational and any applicable regulatory standards.
  • Monitor resident progress, adjust care plans as circumstances evolve, and escalate concerns to the Senior Director of Housing as needed.
  • Track and manage resident data using organizational databases; ensure timely, accurate entry of progress notes, outcomes, and demographic information to support reporting, compliance, and program evaluation.

Resource Coordination & Referrals

  • Collaborate with CCNEK supportive services such as Pregnancy Support Services, Workforce Development and Asset Development.
  • Identify, coordinate, and facilitate referrals to community resources including prenatal and postpartum medical care, mental health counseling, childcare, and housing assistance.
  • Accompany residents to appointments as needed and maintain working relationships with community partners and referral agencies.
  • Transport clients as needed with agency vehicle.
  • Assist residents in obtaining vital documents (birth certificate, ID, Social Security card, etc.).

Resident Support & Programming

  • Facilitate or co-facilitate life skills workshops, parenting classes, and enrichment programming as assigned.
  • Provide crisis intervention, emotional support, and conflict de-escalation within the home.
  • Model healthy boundaries and positive communication in all resident interactions.
  • Participate in house meetings and community-building activities.

Safety & Compliance

  • Conduct room checks and observe house protocols to ensure the physical safety and well-being of all residents and children in the home.
  • Report suspected child abuse, neglect, or domestic violence in accordance with mandatory reporting laws and organizational policy.
  • Adhere to all confidentiality requirements under applicable federal and state law, including HIPAA as applicable.
  • Maintain awareness of and compliance with organizational policies, licensing standards, and any applicable accreditation requirements.

Discharge & Aftercare Planning

  • Coordinate aftercare plans including referrals to transitional housing, continued counseling, parenting support groups, and follow-up case management as available.
  • Maintain appropriate follow-up contact with former residents per organizational protocol.

Administrative

  • Complete all required documentation, statistics, and reports accurately and on time.
  • Attend required staff meetings, supervision sessions, trainings, and in-service events.
  • Participate in on-call rotation as assigned, including evenings and weekends.
  • Perform other duties as assigned by the Senior Director of Housing or Chief Program Officer.

  • Must complete VIRTUS training, background screening, drug testing, and any ongoing training as required

Education, Experience and Expectations

  • Bachelor’s degree in Social Work, Psychology, Human Services, or a related field required.
  • Master’s degree (MSW or related) preferred. Relevant combination of education and experience may be considered.
  • Minimum two (2) years of experience in case management, social services, residential care, or a related human services field.
  • Experience working with pregnant women, mothers, or vulnerable populations strongly preferred.
  • Working knowledge of trauma-informed care, motivational interviewing, and strength-based case management approaches.
  • Familiarity with community resources including prenatal care, social services, housing, and public benefits programs.
  • Strong written and verbal communication skills; ability to document clearly and accurately.
  • Ability to maintain professional boundaries while demonstrating genuine compassion and empathy.
  • Proficiency with Microsoft Office and case management software (training provided as needed).
  • Bilingual (English/Spanish) strongly preferred.
  • Certified Community Health Worker preferred.
  • Must possess a valid driver’s license, dependable transportation, current vehicle insurance and a verified safe driving record.
  • Current CPR/First Aid certification or willingness to obtain within 90 days of hire.
  • Mandatory reporter training as required by state law.

PHYSICAL REQUIREMENTS:

  • Prolonged periods of sitting at a desk and working on a computer, standing, bending and moving
  • Must be able to lift up to 10 pounds
  • May be exposed to infectious diseases
  • Must have physical ability to effectively communicate with others
  • Must have the physical ability and appropriate licensure for driving
