SUMMARY: SUMMARY: Completes psychiatric assessments and collaborates with the interdisciplinary team to develop and implement patient treatment plans. Work with community partners and facilities to coordinate services. Serve as a resource to ensure appropriate utilization of services.
EXPERIENCE: Preferred Behavioral Health Social Work.
SPECIAL SKILLS:Basic Computer Skills
LICENSE/CERT: Required: LMSW (Licensed Master Social Worker), LPN (Licensed Practical Nurse), LCSW (Licensed Clinical Social Worker) within 2 years of hire
EDUCATION: Required: Masters - Social Work (CSWE approved)
