Licensed Practical Nurse (LPN) Home Health Senior Living Facilities
Full-Time | Lee County

Preferred Care Home Health is seeking experienced and compassionate Licensed Practical Nurses (LPNs) to join our growing senior living care team.

As a Preferred Care LPN, you'll have the opportunity to make a meaningful difference in the lives of residents while working alongside a supportive team of nurses, therapists, and other healthcare professionals.

Position Summary
The Licensed Practical Nurse (LPN) provides skilled nursing care to residents within the facility according to the physician's plan of care and under the direction and supervision of a Registered Nurse.

This is an excellent opportunity for an LPN who values resident relationships, clinical quality, independence, and teamwork.

Essential Duties and Responsibilities

  • Provide skilled nursing services in accordance with the physician's plan of care and facility standards.
  • Provide nursing care under the direction and supervision of a Registered Nurse.
  • Administer medications and treatments as ordered.
  • Monitor residents for changes in condition and promptly communicate significant findings to the supervising RN and physician.
  • Accurately document resident care, clinical findings, treatments, and interventions in the medical record.
  • Prepare timely and complete clinical notes in accordance with facility and regulatory requirements.
  • Participate in care conferences, utilization reviews, orientations, in-services, and continuing education.
  • Assist with the development and evaluation of the resident's plan of care in collaboration with the resident, family, representative, and interdisciplinary team.
  • Provide resident and family education regarding medications, treatments, disease processes, and appropriate care techniques.
  • Communicate effectively with physicians and other healthcare professionals involved in the resident's care.
  • Provide resident, family, and caregiver support and education as appropriate.
  • Participate in the facility's Quality Assurance and Performance Improvement (QAPI) program.
  • Maintain compliance with facility policies, procedures, regulatory requirements, and Code of Conduct.
  • Perform other duties as assigned.

Qualifications

  • Active Florida LPN license.
  • Graduate of an approved practical nursing program.
  • Previous senior living or Home Health experience preferred.
  • Current CPR certification.
  • Strong clinical assessment, communication, organization, and documentation skills.
  • Ability to work independently while collaborating effectively with an interdisciplinary care team.
  • Compassionate, professional, and resident-centered approach to care.
