Licensed Practical Nurse (LPN) – Home Health

Full-Time | Home Health

Preferred Care Home Health Services is seeking experienced and compassionate Licensed Practical Nurses (LPNs) to join our growing home health team.

Position Summary

The Licensed Practical Nurse (LPN) provides skilled nursing care to patients in their homes according to the physician’s plan of care and under the direction and supervision of a Registered Nurse.

This is an excellent opportunity for an LPN who values patient relationships, clinical quality, independence, and teamwork.

Essential Duties and Responsibilities
  • Provide skilled nursing services in accordance with the physician’s plan of care and home health certification.
  • Provide nursing care under the direction and supervision of a Registered Nurse.
  • Administer medications and treatments as ordered.
  • Monitor patients for changes in condition and promptly communicate significant findings to the supervising RN and physician.
  • Accurately document patient care, clinical findings, treatments, and interventions in the medical record.
  • Prepare timely and complete clinical notes in accordance with agency and regulatory requirements.
  • Participate in case conferences, utilization reviews, orientations, in-services, and continuing education.
  • Assist with the development and evaluation of the patient’s plan of care in collaboration with the patient, caregiver, representative, and interdisciplinary team.
  • Provide patient and caregiver education regarding medications, treatments, disease processes, and appropriate care techniques.
  • Communicate effectively with physicians and other healthcare professionals involved in the patient’s care.
  • Provide patient, caregiver, and family support and education as appropriate.
  • Participate in the agency’s Quality Assurance and Performance Improvement (QAPI) program.
  • Maintain compliance with agency policies, procedures, regulatory requirements, and Code of Conduct.
  • Perform other duties as assigned.
Qualifications
  • Current and unrestricted Florida LPN license.
  • Graduate of an approved practical nursing program.
  • Previous home health experience preferred.
  • Current CPR certification.
  • Valid driver’s license, reliable transportation, and appropriate auto insurance.
  • Strong clinical assessment, communication, organization, and documentation skills.
  • Ability to work independently while collaborating effectively with an interdisciplinary care team.
