Job Description

We are looking for an RN Clinical Manager who is intuitive and can take on difficult situations with a positive outcome. This individual will need to be very effective in the facilitation of information. You will need to be able to handle a variety of assignments at one time with an equal sense of urgency. This position requires someone who is detail oriented, self-sufficient and able to follow through.
Responsibilities and Duties:
Reviews and evaluates indicated clinical care follow up for patients and communicates with interdisciplinary team to promote quality care.
Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing a clinical plan of care.
Oversees case conference meetings with personnel to facilitate coordination of care and discussion of interdisciplinary group involvement.
Assists Executive Director and other supervisory personnel in the planning, implementation and evaluation of in-service and continuing education programs.
Complies with accepted professional standards and principles.
Participates actively in quality assessment performance improvement program.
Performs other duties and activities as delegated by the Executive Director.
Qualifications and Skills:
Current Texas RN License
Current Certification in BSL/CPR
Valid Texas Driver's License and Automobile Insurance
Previous experience in home care setting with two years management or supervisory experience and a proven ability to work within an interdisciplinary setting
Positive attitude
Ability to work as a team member
Demonstrates high energy level with capability of multi-tasking and self-direction
Has excellent observation, good nursing judgement and communication skills
Demonstrates excellent verbal and written communication skills
Knowledge of WellSky EMR software is a plus
