What You'll Do

As a Program Specialist, you will:

• Maintain a caseload of individuals receiving residential services and ensure person-centered supports are effectively implemented.
• Complete assessments, quarterly reports, annual reviews, outcomes documentation, and other required records to support quality services and regulatory compliance.
• Attend ISP meetings, develop annual outcomes and support plans, and advocate for the needs and goals of individuals served.
• Serve as a liaison between individuals, families, Supports Coordinators, community providers, and agency staff while coordinating services and resources.
• Monitor residential programs, ensure compliance with agency and state standards, participate in quality assurance activities, and support positive outcomes through collaboration with interdisciplinary teams.

QualificationsRequired:

• Associate's Degree (or 60 credit hours) and four (4) years of experience working directly with individuals with developmental disabilities
• Excellent communication, documentation, and organizational skills
• Detail-oriented with strong problem-solving abilities
• Valid driver's license, reliable transportation, current vehicle registration, and vehicle insurance
• Proficiency with Microsoft Office and electronic health record systems
• Ability to work independently and collaboratively with interdisciplinary teams

Preferred:

• Bachelor's Degree and two (2) years of experience working directly with individuals with developmental disabilities
• Experience with person-centered planning, ISP development, and regulatory compliance
• Experience working within residential service programs

Schedule

• Full-Time
• Flexible schedule based on program needs
• Participation in an on-call rotation required
• Regular travel between residential locations and community settings

Physical Requirements

• Ability to lift, push, and/or pull up to 50 pounds
• Ability to sit, stand, walk, bend, reach, stoop, twist, climb stairs, and drive a vehicle
• Ability to travel frequently to residential homes, meetings, and appointments
• Ability to perform essential job functions with or without reasonable accommodation
