Account Manager, ACOs & Medicare Supplement (MediGap) Health Plans

This position is listed on behalf of a partner company, who manages all applications and next steps. Our partner is looking for a Account Manager, ACOs & Medicare Supplement (MediGap) Health Plans based in United States.

This high-ownership role manages strategic healthcare partnerships across ACOs and Medicare Supplement health plans.
You will serve as the primary relationship owner, building trust with clinical, operational, carrier, and executive stakeholders.
The position combines account management, program performance, provider enablement, enrollment growth, and operational coordination.
You will help partners leverage the CMS GUIDE Model to improve engagement and outcomes for people and families affected by dementia.
Success requires translating complex healthcare workflows and performance data into practical actions that drive adoption and measurable results.
You will work cross-functionally with implementation, care coordination, coaching, operations, marketing, and engagement teams.
This is a remote-first opportunity in a fast-moving environment where your work can directly influence how innovative dementia care programs scale nationwide.

Accountabilities:
  • Serve as the primary account owner for assigned ACO and Medicare Supplement carrier partners from post-implementation through ongoing account management and steady-state operations.
  • Build trusted relationships with ACO leadership, clinical leaders, operations teams, care management staff, and physician stakeholders, as well as carrier product, compliance, and member services teams.
  • Develop a strong understanding of the CMS GUIDE Model and communicate relevant workflows to ACO care teams while helping Medicare Supplement partners operationalize the model for their members.
  • Act as the internal liaison for assigned partners, ensuring their needs, priorities, constraints, and expectations are understood and effectively addressed.
  • Support contract adherence, shared-savings alignment for ACOs, GUIDE Model performance objectives for Medicare Supplement partners, and overall partnership health.
  • Own enrollment performance across assigned accounts, monitoring physician and care-team referrals, member outreach, program utilization, outreach effectiveness, and caregiver activation.
  • Identify opportunities to improve enrollment and program adoption through provider education, workflow optimization, targeted engagement, carrier marketing integration, and member communications.
  • Partner with internal teams to identify and resolve enrollment barriers and improve operational processes.
  • Drive physician awareness and buy-in across assigned ACO networks by engaging PCPs, care managers, and practice staff and positioning the caregiver-centric model as a complement to existing care teams.
  • Enable Medicare Supplement member services teams to support enrollment and program workflows effectively.
  • Coordinate patient-, provider-, and carrier-facing materials, training, and communications with Marketing and Engagement teams.
  • Establish and maintain the governance, approvals, workflows, and communication protocols required for proactive outreach to eligible beneficiaries.
  • Coordinate with Implementation, care coordination, coaching, and Operations teams to maintain smooth ongoing delivery across assigned accounts.
  • Support data-sharing processes, including ACO data-file refreshes, Medicare Supplement eligibility exchanges, outreach approvals, and related governance or compliance requirements.
  • Monitor operational SLAs, outreach cadence, member experience, and program quality to ensure consistent and compliant delivery.
  • Lead recurring partner meetings and business reviews, providing clear, data-driven updates on enrollment, engagement, outcomes, savings, and operational performance.
  • Translate complex performance information into concise, executive-ready insights and recommendations for partner stakeholders.
  • Identify opportunities for account expansion and continued growth in collaboration with internal commercial teams.
Requirements
  • Bachelor’s degree in a relevant field.
  • 5+ years of experience in account management, client success, provider relations, payer or carrier relations, partnership management, or a comparable healthcare-facing role.
  • Experience working with ACOs, Medicare Advantage plans, Medicare Supplement (MediGap) carriers, value-based care organizations, or similar healthcare stakeholders.
  • Strong understanding of healthcare operations, provider workflows, population health, and value-based care models.
  • Direct experience with Medicare Supplement carriers, CMS care models, or carrier product and operations teams is highly valued.
  • Demonstrated ability to manage complex relationships involving multiple clinical, operational, commercial, and executive stakeholders.
  • Experience supporting CMS models such as GUIDE, MSSP, ACO REACH, or other value-based care programs is preferred.
  • Familiarity with dementia care, caregiver support, home care, care management, or related healthcare delivery models is an advantage.
  • Experience managing health plan or carrier accounts, including CMS model implementation, member engagement, or related program delivery, is preferred.
  • Strong ability to present performance data, outcomes, and strategic recommendations to executive audiences.
  • Exceptional communication, organization, follow-through, and stakeholder management skills.
  • Strong problem-solving abilities and the capacity to navigate complex operational and relationship challenges.
  • Comfort working in a high-growth, fast-moving startup environment where priorities can evolve quickly.
  • Familiarity with HubSpot is a plus.
