Job Description

Make a meaningful difference for patients and families when compassionate support matters most. The Hospice Social Worker provides emotional support, coordinates resources, and advocates for individuals facing life-limiting illnesses. Working under the supervision of an MSW and alongside the interdisciplinary team, the BSW helps patients and caregivers navigate the social, emotional, and practical challenges of end-of-life care.

As a Hospice Social Worker, you will:

  • Assess patient and family psychosocial needs, support systems, coping abilities, and barriers to care.

  • Collaborate with the supervising MSW and interdisciplinary team to develop and update individualized plans of care.

  • Provide supportive counseling within your scope of practice to help patients and families cope with illness, loss, and caregiving responsibilities.

  • Connect patients and families with community resources, financial assistance, transportation, and other support services.

  • Assist with advance care planning, advance directives, and funeral planning while respecting patient preferences.

  • Identify caregiver stress, safety concerns, and changes in family needs, and communicate concerns promptly.

  • Participate in interdisciplinary group meetings and coordinate services with nurses, physicians, chaplains, and other team members.

  • Support bereavement assessments and referrals in collaboration with the bereavement team.

  • Complete accurate, timely documentation according to agency guidelines.

  • Protect patient confidentiality and honor each family’s cultural, spiritual, and personal values.

Requirements

  • Bachelor of Social Work (BSW) from a program accredited by the Council on Social Work Education.

  • Current social work license or other credentials as required by the state of practice.

  • At least one year of social work experience in a healthcare setting.

  • Ability to provide services under the supervision of a qualified MSW.

  • Hospice, home health, or palliative care experience preferred.

  • Strong communication, organizational, and problem-solving skills.

  • Compassionate approach to working with patients experiencing serious illness and families facing grief and loss.

  • Valid driver’s license, reliable transportation, and current auto insurance for patient visits.

  • HomeCare HomeBase experience preferred.
