The Social Worker BSW intervenes with patients who have complex psychosocial needs, economic co-morbidities, require assistance with eligibility determination for social programs and funding sources, and qualify for community assistance. To promote patient-centered care, the Social Worker BSW provides care that is respectful of and responsive to individual patient preferences, needs, and values, and ensures that patient values guide all decisions. Supports compliance with treatment protocols, and assists with resource linkage as part of a multidisciplinary healthcare team.

- Bachelor's degree in Social Work; no experience required. Must be licensed to practice as a Social Worker in Texas (LBSW).
Preferred Qualifications:
- Experience or training in an Infectious Disease (ID) clinic
- Experience working in a Ryan White-funded facility

ESSENTIAL JOB FUNCTIONS As applicable to patient population
- Psychosocial Assessment and Intervention
- Performs comprehensive medical and psychosocial assessments to identify barriers to care.
- Implements and monitors individualized care plans and conducts follow-up to support retention in care.
- Assesses psychosocial risk factors including support systems and coping ability.
- Provides interventions addressing emotional, social, financial, and environmental challenges.
- Advocates for patient and family empowerment and access to healthcare services.
Counseling and Support
- Provides emotional support and conflict resolution for patients and families.
- Applies knowledge of family systems in crisis intervention.
Communication and Collaboration
- Communicates effectively with patients, families, and interdisciplinary teams.
- Maintains professionalism and cultural competency.
- Collaborates with healthcare providers to coordinate care.
- Utilizes problem-solving skills in patient care situations.
Documentation
- Completes timely and accurate assessments, care plans, and documentation.
- Ensures compliance with HIPAA, program requirements, and UTMB policies.
Time Management
- Completes assessments and interventions in a timely manner.
- Responds promptly to requests for social work services.
- Prioritizes caseloads based on acuity.
Performance Improvement
- Participates in quality improvement initiatives.
- Aligns with UTMB mission, vision, and policies.
- Adheres to internal controls and reporting structure
Salary Range:

WORKING ENVIRONMENT/LOCATION OF POSITION
- Hospital, ambulatory care, or community environment.
- Moderate physical effort may be required, such as walking, standing, and lifting materials, equipment, and/or objects
