Job Title: Community Health Worker
Reports To: Program Manager
Department: Health Center – Enabling Services
FLSA Status: Hourly, Non-Exempt
SUMMARY
The Community Health Worker operates under indirect supervision and reports to the Program Manager, serving as a vital link between healthcare teams and clients. Working across clinical and community settings, including home visits, this role collaborates with comprehensive care teams (medical, behavioral health, and dental) to improve client health outcomes. The position focuses on connecting clients to essential resources, providing health education, and coordinating care services to enhance overall client well-being.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Coordination of Support and Enabling Services
- Assist clients across home, community, and clinic settings
- Educate clients about program benefits and health impact
- Assess non-medical factors affecting health (e.g., insurance status, food, utilities, housing, education, employment, financial, transportation, safety, clothing, child care, social/emotional health, etc.)
- Coordinate client referrals to address identified needs
- Develop personalized support plans
- Provide client advocacy
- Help clients overcome barriers to accessing comprehensive care (medical, behavioral health, dental)
- Support proper utilization of health center services
Eligibility & Enrollment Assistance
- Process applications and renewals for:
- Local, State, and Federal Programs and Resources
- Medicaid, CHIP, SNAP, and TANF
- Health Center Grant Programs, i.e., Sliding Fee Scale (FQHC), Primary Health Care (PHC)
- Prescription Assistance Programs (PAP)
- Affordable Care Act coverage
- Medicare/Medicare Advantage plans
Documentation & Reporting
- Document all encounters, assessments, and outcomes
- Maintain detailed electronic client records
- Complete monthly activity reports
- Track client compliance with plan objectives
Health Center Outreach
- Coordinate and/or conduct health education presentations
- Participate in health center outreach activities, including but not limited to: health promotion events, community clinics, health and resource fairs, food distributions, community events
- Travel to various community locations
- Promote awareness of health center services and available assistance programs
General
- Adhere to all Chambers Health appearance standards and always maintain a professional dress while working.
- Maintains friendly, efficient, positive customer service attitude toward customers, clients and co-workers. Is responsive to customer needs.
- Uses Service Excellence model when interacting with patients, client, and guests.

- Maintains compliance with Chambers Health standards of operation. Always adheres to Chambers Health Business Conduct Policy. Maintains all records and reports to ensure compliance with all local, state and federal regulations and codes.
- Maintains compliance with all requirements of Federal, State and local regulations and guidelines including the Civil Rights Act of 1964, as amended, the Age Discrimination in Employment Act of 1967, as amended, and the Americans with Disabilities Act, as amended.
OTHER DUTIES
- Performs other related duties as may be assigned by Program Manager and Director of Grants Management and Community Programs.
ADDITIONAL INFORMATION
- Travel: Frequent; Must be in possession of a valid Texas driver’s license is a requirement for this job.
- Work Week: Primarily Days & Occasional Evenings / Weekends

QUALIFICATIONS
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. - Community Health Worker (CHW) certification, or ability to obtain certification within the first 6 months of employment.
- Certified Application Counselor (CAC) certification, or ability to obtain certification within first 6 months of employment.
- Bilingual (Preferred)
- Strong interpersonal and communication skills and the ability to work effectively with clients in a diverse community.
- Knowledge of community agencies and resources
- Working Knowledge of Microsoft Office programs (Word, Excel, PowerPoint, Outlook) and Electronic Health Record (EHR).
- High School Diploma or GED
- At least one (1) year of applicable experience
