Northeastern Vermont Regional Hospital (NVRH) is seeking a Patient Outreach & Quality Coordinator to join our Kingdom Internal Medicine (KIM) team. This is an excellent opportunity for a detail-oriented professional who enjoys working with data, analyzing trends, improving processes, and supporting high-quality patient care. In this role, you'll help drive quality improvement initiatives, support Patient-Centered Medical Home (PCMH) efforts, and use data to identify opportunities that improve patient outcomes and practice performance. Your work will directly impact the health and well-being of patients throughout our community.

About This Role

As the Patient Outreach & Quality Coordinator, you will develop, coordinate, implement, maintain, and evaluate quality improvement activities within Kingdom Internal Medicine. Working closely with providers, staff, information systems teams, and practice leadership, you will collect, analyze, and interpret data to identify improvement opportunities, support outreach initiatives, and enhance the overall patient experience. This position is ideal for someone who enjoys combining healthcare operations, data analysis, project coordination, and patient-focused process improvement in a collaborative team environment.

Key Responsibilities

  • Utilize electronic medical record (EMR) data, patient registries, and reporting tools to identify trends, monitor quality measures, support patient outreach efforts, and recommend opportunities to improve patient outcomes and practice performance.
  • Facilitate quality improvement initiatives by analyzing workflows, leading data-driven improvement projects, coordinating quality meetings, evaluating outcomes, and collaborating with providers and staff to optimize patient care and operational efficiency.
  • Support Patient-Centered Medical Home (PCMH) activities by tracking quality measures, preparing quarterly and annual reports, coordinating required data submissions, and assisting with patient engagement and outreach initiatives.
  • Coordinate patient transfers into and out of the practice, ensuring seamless transitions of care, timely acquisition and release of medical records, accurate provider panel management, and clear communication with patients, providers, and external healthcare organizations. Support administrative workflows including chart reviews, referral management, and patient data maintenance.

Required Qualifications

  • High school diploma or equivalent required.
  • Minimum of two years of experience in a hospital, healthcare, or medical office setting preferred.
  • Experience using computer systems and electronic health records.
  • Knowledge of medical terminology.
  • Ability to manage multiple priorities and remain calm and professional in a fast-paced environment.
  • Strong written and verbal communication skills.

Preferred Skills

  • Experience working with healthcare quality measures, data analysis, or performance improvement initiatives.
  • Strong analytical and problem-solving skills.
  • Advanced proficiency with Microsoft Excel, reporting tools, and database systems.
  • Ability to organize, interpret, and present data in a meaningful way.
  • Strong attention to detail and commitment to data accuracy.
