Responsibilities:

Perform in-home nursing assessments

Develop and manage patient care plans

Provide skilled nursing care including wound care, medication management, infusion therapy, catheter care, and palliative care

Educate patients and caregivers

Coordinate care with physicians and interdisciplinary team

Document care accurately in EHR

Follow safety and infection control standards

Qualifications:

Active RN license

2–3 years nursing experience (home health preferred)

Strong clinical and communication skills

EHR experience

Valid driver’s license and reliable transportation
