Job Description
TThe Elder Care Program Coordinator supports Tribal Elders by conducting home visits, assessing needs, and connecting them to medical, social, and community resources. This role serves as a liaison between Elders, their families, and service providers to ensure needs are met and services are coordinated. The coordinator maintains documentation, supports care planning, and builds relationships within the Tribal community.
Essential Duties:
Level I -
- Conduct home visits and welfare checks to evaluate and monitor Tribal Elders and their needs. Identifies economic, social, cultural, physical, and environmental factors that impact functioning.
- Evaluate the effectiveness of the care plans and recommend adjustments as needed.
- Maintain outreach and provide support services to Tribal Elders based on individual needs.
- Conduct Activity of Daily Living Assessments to Determine eligibility for program services.
- Connect Tribal Elders with essential community resources, providing hands-on, in-person assistance as needed.
- Prepare and submit payment requests and Elder In-Home Care Timesheets for supervisor review and approval.
- Assist Tribal Elders with navigating resources such as Washington Connections, Apple Health and DSHS Long-Term Services and Supports; while collaborating with medical professionals and other service providers to support coordinated care.
- Coordinate transportation for health-related appointments as needed.
- Maintain transportation logs, reports, and related program documentation.
- Prepare purchase requests for program services, supplies, and activities for program manager review and approval.
- Participate in Elder program activities and events in a respectful and supportive manner.
- Maintain accurate client records, case notes, and other required program documentation.
- Participate in department meetings and trainings as appropriate.
- Assist Tribal Elders’ physical and mental wellness, needs, preferences, and abilities to develop individualized care plans.
- Work with Tribal Elders, their support networks, and health care professionals to implement individualized care plans.
- Document Trial Elders’ progress, including charting referrals, home visits, and other notable interactions
- Evaluating Tribal Elders’ progress periodically and make adjustments to care plans or services as needed to improve outcomes.
Level II -
Performs all duties of Level I, plus:
- Assist with program policy development as needed.
- Utilize Tribal Assister certification to assist Elders with enrollment in Apple Health and other eligible assistance programs.
Additional Duties:
- Assist in identifying Tribal Elder needs to ensure optimal support and care.
- Participate in program staff meetings, provider meetings, and workgroups as needed.
- Attend required trainings and continuing education necessary to effectively perform job duties.
- Assist with Title VI Elder Needs Assessment Survey and other surveys for information.
- The Tribe values its commitment to community service and the well-being of its members. In recognition of this, each employee may be expected to perform duties from time to time which may not be related to their regular responsibilities.
- Other duties as assigned.
- Proficient in computer skills including Microsoft Office applications.
- Proficient in navigating DSHS resources and applications.
- Knowledge of Medicare and Social Security Administration resources and benefit programs.
- Case management experience
- Strong organizational skills with attention to detail and accuracy.
- Ability to prioritize workload and meet strict deadlines.
- Excellent customer service skills and written and verbal communication.
- Ability to communicate effectively with diverse populations in a courteous, clear, and helpful manner. Knowledge of Native cultures, history, and traditions preferred.
- Knowledge of age-related health and social conditions affecting seniors preferred. Ability to maintain confidentiality and use tact and discretion.
- Demonstrate professionalism, strong communication skills, and sensitivity to the needs of Tribal Elders and their families.
Education & Experience Requirements:
Education Required:
Level I -
- Associate degree in human services, social work, healthcare administration, public health, nursing, or a closely related field preferred; or High School Diploma/GED with directly related experience in elder care, healthcare support, or human services is required. HCA (Home Care Aide) or CNA (Certified Nursing Assistant) certification preferred
Level II –
- Bachelor’s degree in social work, public health, human services, or related field; or an equivalent combination of education and experience that demonstrates the ability to perform the essential functions of the position may substitute for degree requirement.
Experience Required:
Level I -
- Minimum of two (2) years of experience working with senior citizens is required.
- Minimum of one (1) year of experience with case management is required
- Minimum of one (1) year of experience working in food preparation is preferred.
- Minimum of two (2) years of experience working in Native Community with Elders or Native families is preferred.
Level II –
- Minimum of three (3) years of experience working with senior citizens is required.
- Minimum of two (2) years of experience working in food preparation is required.
- Minimum of two (2) years of experience supporting aging-related health conditions or in geriatric social work.
- Minimum of three (3) years of experience working in a Native Community with Elders or Native families is required.
Certifications/Licenses:
Level I -
- Must have and maintain a valid Washington State driver’s license.
- Current CPR/AED Certification.
- Must obtain a valid Food Handlers Certification within 90 days of employment.
Level II –
Requires the certifications/license requirements of level I; plus:
- Must have and maintain a Certified Tribal Assister certification.
Other Requirements:
- Must be able to pass a thorough pre-employment background investigation.
- Must agree to be subject to the Tribe's Controlled Substance and Alcohol Testing Policy to include pre-employment screening.
- Must be able to successfully pass the Tribe’s Defensive Drivers Class.
Work Environment:
The work is performed in both an office setting and in the field, including frequent travel by car to conduct home visits and provide services to Tribal Elders.
Tribal Preference:
The Snoqualmie Indian Tribe follows the Tribal Hiring Preference as outlined in tribal policies, including the Tribal Employment Rights Ordinance (TERO). TERO sets forth certain hiring preferences for qualified individuals who are enrolled in a federally recognized Indian tribe, as well as, in some cases, preference for certain family members of those individuals.
Additional Information:
- This is a full-time hourly position and employment is at-will.
- Compensation will be determined based on experience and starts at $27.21 per hour.
