Position Title: RN - Coordinator Stroke Program (1.0 FTE, Days)

Description

Wage range: $96,366.40 - $ 154,211.20

Posted wage ranges represent the entire range from minimum to maximum. For jobs with more than one level, the posted range reflects the minimum of the lowest level and the maximum of the highest level. Some positions also offer additional premiums based on shift, certifications or degrees. Job offers are determined based on a candidate's years of relevant experience, level of education and internal equity.

Job Summary: This position holds a critical role in the day-to-day coordination of the Stroke Center. In partnership with the Stroke Program Supervisor and Medical Director, this position is responsible for planning, implementing, and coordinating services and activities associated with the Stroke Center. This includes but is not limited to, collection, analysis, and submission of regulatory data.

Primary Duties:

1. Performs day-to-day review of patient records for various Stroke Center activities/requirements including concurrent review for compliance with policies and clinical practice guidelines. Provides feedback as needed.

2. Collects, reviews, and analyzes process and outcomes data from a variety of sources (chart review, computer generated reports, clinic reports, and observation) to provide accurate and consistent information for review and evaluation of patient care outcomes and assessment of the Stroke Performance Measures.

3. Monitors quality, continuity of care, clinical outcomes and system issues related to care delivery, developing quality audits, case reviews, identifying trends and sentinel events.

4. Facilitates and designs systems for accurate data collection, feedback, analysis, and trend reports while protecting confidentiality.

5. Responsible for ensuring that the medical records abstracted are complete and accurate as per DNV CSC performance guidelines and assisting in the submission before established deadlines to DNV and AHA- Get With The Guidelines (GWTG).

6. Maintains the stroke registry for the purpose of regulatory standards and performance improvement.

7. Validates the accuracy of final coded diagnoses related to stroke and escalates encounters appropriately when discrepancies are identified.

8. Analyzes and presents data as needed to Stroke Program leadership for ongoing internal projects and external reporting requirements.

9. Provides timely data for ED, Hospitalist and Stroke Center dashboards related to the Stroke Performance Measures.

10. Responsible for communicating & collaborating with ED and Stroke Unit managers to provide feedback and case reports for staff caring for stroke patients.

11. Collaborates with and provides ongoing education and support to the Qcentrix data abstraction teams for Kirkland and Monroe campuses to ensure data integrity and timely abstraction for all stroke core measures.

12. Collaborates with the Medical Director to provide performance measurement feedback reports to the Medical Staff office and appropriate Medical Directors regarding physician compliance with quality metrics in the care of stroke patients.

13. In partnership with Stroke Program Supervisor, participates in performance improvement activities in accordance with organizational plans using various methodologies.

14. Works closely and meets regularly with the Stroke Program Supervisor and Medical Director to review stroke cases, participate in Stroke Core Team Huddle and implement changes to help improve Stroke Program outcomes.

15. Participates in and provides data presentations for the Stroke Interdisciplinary Committee quarterly meetings.

16. Assists with the training and development of staff and ensures competency standards are met for the care of stroke patients as needed.

17. Participates in inter-facility program feedback and development including Evergreen Health Redmond and Monroe campuses.

18. Monitors pre-hospital care and triage patterns for consistency with established protocols. Assists in EMS pre-hospital stroke education and feedback as needed. Provides case reports on all treatment cases to regional fire department MSOs.

19. Performs other duties as assigned.

License, Certification, Education or Experience:


REQUIREDfor the position:

Bachelors degree in nursing (BSN) or other health related field

Licensed in the State of Washington as a Registered Nurse

5 years of experience in a hospital setting

Ability to use qualitative and quantitative data

Knowledge of Performance Improvement tools and methodologies


DESIREDfor the position:

Masters degree in Nursing (MSN) or other health related field

Direct patient care experience

Experience with spreadsheets and QI data display software
