JOB SUMMARY

The Community Outreach Coordinator is an integral part of the marketing and growth team and is responsible for bringing the ConcertoCare model of care to seniors with complex needs. Under the direction of the Growth Director, the primary focus of the Community Outreach Coordinator is the development and maintenance of strong relationships with senior living referral partners.

As a key part of the team, the Community Outreach Coordinator identifies and cultivates new relationship opportunities that will support the organization's goals for enrollment growth and generate high-quality referrals to the enrollment team. This position serves as the liaison between the community and the practice and educates partners, potential participants, and caregivers.

SUCCESS IN ROLE

Success in this position is measured by patient enrollment to ConcertoCares mobile primary care services and meeting or exceeding established weekly and monthly enrollment and outreach goals. This includes meeting performance targets on enrollment and productivity (i.e. partner touches, drop-ins, educational/onboarding presentations).

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Reside or be willing to relocate to the Puget Sound or Spokane area of Washington state.
  • Meets monthly enrolled census targets by generating enrollment referrals to meet growth and outreach productivity targets.
  • Develops relationships with new, potential referral partners and maintain existing partnerships.
  • Participates in all marketing, outreach, and community engagement activities.
  • Facilitates communication between partner senior living communities and clinical staff.
  • Maintains a thorough knowledge of ConcertoCare services, capabilities, and provider relations.
  • Provides training and education to senior living communities and referral partners on services and capabilities.
  • Gauges facility/partner satisfaction and resolves customer service issues.
  • Adheres to established outreach cadence to referral partners to include phone, email, and in person visits.
  • Other duties as assigned.

QUALIFICATIONS

  • 2+ years sales and outreach experience in a healthcare context. Outreach to geriatric population a plus.
  • Referral network development experience.
  • Communication skills: Exceptional public speaking, communication, and customer service skills.
  • Effective oral and written communication skills.
  • Energetic, well organized, dependable, flexible, and resourceful.
  • Comfort with customer relationship management (CRM) tools.
  • Familiarity and comfort with Microsoft Suite.
  • Must possess a valid Washington state driver's license, reliable transportation, and insurance.
  • Ability and means to travel as needed in a timely manner to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required. DRIVER REQUIREMENTS: Licensed for a minimum of 5 years. No vehicle-related suspensions/reinstatements, DUI, reckless driving, or felony convictions within the last 7 years. Multiple violations and accidents within the last 5 years would be subject to review.

Base Salary/ Wage Range $60,000 to $65,000 annually plus bonus. Compensation for the role is commensurate with the candidate's qualifications, skills, competencies, and experience and may fall outside of the range shown.
