Job Description
Community Living, Inc. is looking for a Vocational Case Manager of Assessment to help participants discover their strengths, interests, and employment potential. In this role, you’ll work directly with individuals to assess skills, identify support needs, and help create meaningful pathways toward employment and independence.
This position is a great fit for someone who enjoys building relationships, and making a direct impact in the community. Every day looks a little different, from meeting with participants and families to visiting job sites, conducting assessments, and collaborating with local employers.
Pay Rate: $23.25/hr
Schedule: FT 40 Hrs. Monday - Friday days and evenings depending on client availability
- Conduct vocational assessments, interviews, and evaluations to help determine employment goals and job compatibility
- Meet with participants, families, referral sources, and support teams to gather information and build individualized employment recommendations
- Coordinate caseloads, documentation, and individual plans through the electronic documentation system
- Visit community job sites and employment partners to assess opportunities and participant success
- Observe and document participant skills, accommodations, and support needs in real-world work environments
- Develop and maintain positive relationships with local employers and community partners
- Attend intake and pre-service meetings for new participants
- Ensure compliance with CARF, DVR, DMH, DDRB, and agency standards
- Maintain billable hours and complete all required trainings
- High school diploma or GED required; Bachelor’s degree in social services or related field preferred
- Experience working with individuals with intellectual and/or developmental disabilities preferred
- Strong communication, organization, and problem-solving skills
- Ability to manage multiple caseloads and priorities effectively
- Comfortable working independently and as part of a team
- Experience with Microsoft Office programs including Outlook, Word, and Excel
- Valid driver’s license required
