MISSION & VISION STATEMENT:
The Kewa Pueblo Health Corporation (KPHC) is established for the purposes of carrying out the vision and mission of the Santo Domingo Health Center (SDHC). POSITION PURPOSE:
The purpose of this position is to provide direct patient care at the Santo Domingo Health Center, including collaborating with community-based health personnel and coordinating health care with health providers and the Santo Domingo Pueblo Health Programs.
PERFOMANCE EXPECTATIONS:
In performance of their respective tasks and duties all employees of the Kewa Pueblo Health Center are expected to conform to the following:
ESSENTIAL DUTIES, FUNCTIONS & RESPONSIBILITIES:
Treats full range of family practice cases, referring only the more difficult specialty cases and those requiring in-patient care services.
Performs family practice diagnostic and treatment procedures independently.
Functions as part of the health care team along with the PHN, Pharmacy Practitioner, P.A., Dental Staff, Clinical Nurse, Clerk, Nutritionist and Health Educator, as well as other community-based health personnel (i.e. CHR, WIC, EMT’s, etc.).
Participates in on-going training of Health Center staff.
Coordinates medical care provided in the Santo Domingo Pueblo.
Maintains communication with Santo Domingo Pueblo members and tribal officials, providing medical representation at the Tribal and Community meetings, as required.
Maintains patient records in the Problem Oriented Medical Record format utilizing Electronic Health Records (EHR).
Provides patient education on specific treatment as well as in general preventive care.
Reviews patient charts for quality of care and performs other quality assurance assessments in the Santo Domingo Health Center.
Provides reports to the medical staff regarding Santo Domingo health Center activities, needs and work data.
Assists Chief Medical Officer in arranging for health assessments for specific groups (i.e. Head start, Preschool, School Sports, and Firefighters). Assesses health maintenance needs for chronic groups (i.e. diabetic, hypertensive, pediatric and adult medicine).
Provides care to prenatal, well child, post-partum and other special patient groups.
Maintains a current professional certification and licensure to practice through participation in appropriate programs of continuing education.
Observes and abides by the Medical Staff by-laws of the Kewa Pueblo Health Corporation and the Rules and Regulations of the Medical Staff.
Completes, appropriately utilizing Electronic Health Records (EHR), data entry of each patient visit(s) and completes and/or records on other required forms.
Adheres and complies with all Santo Domingo Health Center’s policies and procedures, rules and regulations.
Coordinates leave arrangements with other clinical providers so as to have minimal impact on the delivery of health care services.
Performs other related duties as assigned.

The work environment characteristics described here are representative of those an employee encounters while performing the primary functions of this job. Normal office conditions exist, and the noise level in the work environment can vary from low to moderate. In addition, work is performed in a clinic or field clinic setting with continued exposure to patients having a variety of illnesses and communicable diseases. This position may be exposed to certain health risks that are inherent when working within a health center facility.
PHYSICAL DEMANDS:
The physical demands described here are representative of those that must be met by an employee to successfully perform the primary functions of this job. While performing the duties of this job, the employee may be required to frequently stand, walk, sit, bend, twist, talk and hear. There may be prolonged periods of sitting, keyboarding, reading, as well as driving or riding in transport vehicles. The employee must occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this job include reading, distance, computer, and color vision. Talking and hearing are essential to communicate with patients, vendors and staff.
MENTAL DEMANDS:
There are a number of deadlines associated with this position. The employee must also multi-task and interact with a wider variety of people on various and, at times, complicated issues.
