Job Description

We are hiring a Licensed Practical Nurse (LPN) to serve as an Assisted Living Manager for our small, residential-style assisted living facilities. This role provides clinical oversight, staff supervision, and regulatory compliance across four homes located within the same city. Ideal for an LPN who enjoys leadership and a hands-on care environment.

Responsibilities

• Provide nursing oversight in compliance with Minnesota Assisted Living Licensure (144G)

• Conduct resident assessments and monitor health changes

• Oversee medication administration, documentation, and audits

• Supervise, train, and support caregivers and medication aides

• Coordinate care with providers, pharmacies, residents, and families

• Ensure accurate documentation and DHS compliance

• Prepare for surveys, audits, and inspections

• Travel between four nearby group-home locations

Qualifications

Required:

• Active Minnesota LPN license

• Assisted living, long-term care, or group home experience

• Knowledge of MN assisted living regulations (144G)

• Strong leadership and communication skills

• Valid driver’s license and reliable transportation

Preferred:

• Supervisory or management experience

• Experience in multi-site or group-home settings

• EHR experience

• CPR/First Aid certification
