Job Summary:
The primary function of the hospice intake coordinator is to facilitate the referral intake process, coordinate care with the interdisciplinary team, the patient, their family, and the referral source.
Responsibilities:
- Receives and appropriately documents new referrals in a timely manner.
- Participates in coordinating care with Nursing Supervisors, patients, and the Interdisciplinary Group/ Team.
- Provides effective communication to patients, staff members, other health care professionals, and referral sources.
- Responds to the Hospice’s and patient needs in a professional and creative manner.
- Demonstrates commitment, professional growth, and competency.
- Participates in the Hospice’s sponsored in-service trainings.
- Participates in the Hospice’s QAPI program.
- Promotes the Hospice’s philosophy and administrative policies to ensure quality of care.
Experience:
Hospice/Home Health experience preferred.
High school diploma or equivalent.
Skills:
Excellent organizational and time-management skills.
Strong communication and interpersonal skills.
Proficiency in scheduling software and electronic health records (EHR) systems.
Ability to work independently and as part of a team.
Compassionate and empathetic demeanor towards patients and families.
Transportation:
Reliable transportation and valid auto liability insurance.
