R.N. Case Manager, Hospice in Clinton, Missouri at Golden Valley Memorial Hospital

Performs services reasonable and necessary for treatment of an illness or injury in the home setting. Services include observation and assessment, teaching and training, management of a care plan, and assistance with medications and treatments. The RN will have an assigned patient caseload with planning, scheduling, and documentation responsibilities and an assigned geographical territory. Will be expected to assist with other GVMH home programs. Treats patients with all types of disabilities and multiple medical problems. Will assist with orientation of new staff and training of students. Participates in call rotation.

Requirements:
Graduate of an accredited school of nursing
Current Missouri RN license in good standing
BLS certification
Current MO driver's license and car insurance
Registration with the Missouri Family Care Safety Registry with no findings.
One year experience
Hospice or home care experience preferred.
