Licensed Practical Nurse (LPN)

Location: Alluvion Health – 601 1st Ave N, Great Falls, MT
Full-Time | Non-Exempt | 40 Hours per Week
Schedule: Monday–Friday, typically 8:00 a.m.–5:00 p.m.

Alluvion Health is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our Medical team at our main clinic located at 601 1st Ave N in Great Falls, Montana.

In this role, you’ll be an important part of an integrated primary care team, working alongside providers and other healthcare professionals to deliver high-quality, patient-centered care to individuals and families throughout our community.

What You’ll Do

As an LPN at Alluvion Health, you’ll support patients throughout their care experience while helping our providers and care teams deliver safe, efficient, and compassionate care.

Your responsibilities will include:

  • Provide practical nursing care within your scope of licensure.
  • Screen patients and gather appropriate clinical information.
  • Review and update patient medical records prior to visits.
  • Prepare patients for examinations, procedures, treatments, and testing.
  • Assist providers with examinations, procedures, and other patient care needs.
  • Administer medications, injections, immunizations, treatments, and testing as ordered and within scope of practice.
  • Provide patient education and support patients in understanding their care plans and treatment recommendations.
  • Document patient encounters, progress, medications, treatments, referrals, and follow-up care accurately in the electronic medical record.
  • Coordinate patient flow to help ensure patients receive timely care.
  • Support patient engagement, education, care planning, and follow-up.
  • Coordinate and track referrals for internal and external services.
  • Facilitate communication between patients, providers, specialists, community resources, and other members of the care team.
  • Participate in care team huddles, meetings, Quality Improvement activities, peer review, and committees as assigned.
  • Maintain patient confidentiality and comply with HIPAA and HITECH requirements.
  • Work collaboratively with providers, staff, patients, families, and community partners.
  • Maintain positive, respectful, and professional working relationships.
  • Follow Alluvion Health policies, procedures, and clinical protocols.
  • Assist with special projects and other duties as assigned.

  • A commitment to confidentiality, patient privacy, and quality care.
Education & Qualifications
  • Licensed Practical Nurse degree required.
  • Current State of Montana Licensed Practical Nurse license required.
  • Three to five years of experience in a primary healthcare setting preferred, or an equivalent combination of education and experience.
  • Valid driver’s license required.
  • Must be fluent in English and able to effectively read, analyze, document, and communicate complex or sensitive information.
