Job Description

JOB SUMMARY:
  • Conduct an initial psychosocial assessment of the patient within 30 days of admission, with a reassessment to be performed 90 days after the first assessment and prepare the necessary documentation.
  • Develop treatment and psychosocial goals for the initial care plan and update the plan as outlined by the clinic’s governing body for both stable and unstable patients.
  • Complete and document psychosocial evaluations quarterly and annually; record the results.
  • Complete a quality-of-life measure for each patient in accordance with LifeBridge standards.
  • Notify the multidisciplinary team of the patient's psychosocial issues impacting treatment.
  • Analyze and provide information, facilitating referrals to community, private, state, and federal agencies based on psychosocial needs, such as financial assistance programs, medication support, Vocational Rehabilitation, transportation resources, mental health and chemical dependency services, and home/homemaking agencies.
  • Counsel new and current patients and their families on treatment options and modalities (e.g., transplantation, PD); offer support and assistance with treatment planning; help patients and families adjust to dialysis and lifestyle changes and provide resource information.
  • Advocate for patients and families in accordance with LifeBridge Health's ethical standards. Facilitate or refer patients and families to support and education groups as needed.
  • Coordinate and participate in patient/family care conferences with the multidisciplinary team. Offer crisis intervention services as required.
  • Respond to patient concerns and assist with health insurance questions. Answer patient inquiries regarding Medicare and Medicaid.
  • Aid patients with billing questions.
  • Collaborate with Financial Services and the agency to address questions about commercial insurance. Help patients with financial issues by applying for grants from organizations such as the National Kidney Foundation, the American Kidney Fund, state renal programs, the Veterans Administration, and others.
  • Comply with policies and procedures and adhere to applicable state and federal regulations for patient care and services (including Medicare, OSHA, HIPAA, OIG, and other governing bodies).
  • Participate in and comply with injury prevention and safety training programs mandated by state or federal regulations.
  • Maintain patient confidentiality in accordance with HIPAA.
  • Attend monthly Quality Assurance Performance Improvement meetings, patient care conferences, unit meetings, and in-service training.
  • Perform other duties as assigned or required. Work hours as needed to ensure quality patient care.
  • Attend team meetings, conference calls, and training sessions as necessary.
  • Know, understand, and follow patient and staff guidelines, employment policies, hospital or organizational procedures, or company procedures.

REQUIREMENTS:

  • Master's Degree in Social Work from accredited school.
  • Current LCSW license in Maryland is required for this position.
  • Minimum of 3 years of experience required; 5+ years of experience preferred.
