Position Description
The Case Manager provides direct support to participants at the Casa Milagrosa Day Resource Center in Los Angeles by conducting intake assessments, identifying needs, and developing individualized support plans. The role connects participants to housing, healthcare, employment, counseling, and other community-based services while promoting stability and self-sufficiency. The Case Manager maintains case documentation, tracks participant progress, and prepares reports as required. The position works closely under the day-to-day supervision of the Program Manager and operates under the broader oversight of the Los Angeles City Director, collaborating with internal staff and external partners to coordinate services and strengthen community linkages in alignment with Depaul USA values.
Primary Duties and Responsibilities:
Essential Job Functions
• Responsible for conducting interviews to determine client needs and develop plans for obtaining needed services at Casa Milagrosa.
• Will demonstrate direct assistance, coordination of care, and linkage to the appropriate services while developing support plans for clients, and s/he will report to and work closely with the Program Manager.
• Promotes and encourages client participation in community activities. Coordinates and refers clients to counseling, case management, medical, and other appropriate services.
• Builds professional relationships with clients and assists them with difficult or problematic situations in a sensitive manner.
• Completes intake documentation for new clients and conducts formal assessments to determine service eligibility, ensuring accurate tracking of enrollment, activities, and required reporting.
• Responsible for documenting case management notes in New Org, including client activities, progress, and recommendations.
• Maintains accurate HMIS and agency-required reporting.
• Supports clients in accessing referred services and progressing toward greater independence and stable housing in alignment with Depaul USA values and practices.
• Assists clients with the necessary steps to gain employment and provides housing navigation support, including referrals to housing resources and supportive housing opportunities through Depaul USA in Los Angeles.
• Assists clients with difficult or problematic situations in a sensitive manner while ensuring all appropriate records and forms are completed.
• Provides crisis intervention and de-escalation support to clients as needed in a sensitive manner.
• Develops and implements a process for clients to build life skills development.
• Collaborates with colleagues to develop client support plans and ensure accurate documentation in case files.
• Collaborate with other agencies in the community and participate in coalition meetings, in coordination with the Program Manager and under the guidance of the Los Angeles City Director. Builds good working relationships with other community stakeholders, programs, services, and agencies.
• Participates in weekly/monthly staff meetings to update staff on client progress and areas for development. Coordinates work with other staff members.
• Communicates client needs, program trends, and operational concerns to the Program Manager and escalates broader system issues to the Los Angeles City Director as appropriate.
Additional Tasks and Responsibilities
• Assists with Casa Milagrosa Day Center’s opening procedures and daily operations.
• Conducts tours of the facility for visitors and volunteers, may supervise volunteers.
• Prepare reports, statistics, and other papers required by the Program Manager and the Los Angeles City Director, ensuring appropriate escalation of programmatic trends or concerns as needed.
General Responsibilities
• Stays informed of current local and national events, trends, and issues (wherever possible) that impact Depaul USA’s work with the homeless.
• Strives continuously to develop the role in collaboration with the supervisor to ensure that all tasks are undertaken effectively and appropriately, meeting the strategic goals and objectives of Depaul USA.
• Participates in internal/external meetings as required and attends training events, conferences, and other functions as necessary. Participates in regular supervision and annual appraisal and helps in identifying your job-related development and training needs.
• Ensures adherence to all Depaul USA policies and procedures, particularly those relating to Health and Safety, Safeguarding, Code of Practice, and Confidentiality.
• Contributes to the effective implementation of Depaul USA’s Equal Opportunities Policy as it affects both the organization and its work with this client group.
• Undertakes the role professionally, always maintaining a high-quality work standard and always working in accordance with the Vision, Mission, and Values of Depaul USA.
• Undertakes any other duties that may be required which are commensurate with the role.
Qualifications
The ideal Case Manager will be an experienced human services professional with experience supporting individuals experiencing homelessness and housing instability. The role requires strong skills in intake assessment, case management, housing navigation, and coordination of services across behavioral health, housing, and community-based systems. Experience working in a day resource center or similar low-barrier service environment is highly preferred. The ideal candidate will also demonstrate an understanding of the homelessness response system, including coordinated entry, supportive housing pathways, and community-based service networks, and possess the following:
• Bachelor’s degree in social work, human services, psychology, or a related field preferred, with at least two years of experience working with individuals experiencing homelessness and/or complex social service needs.
• Experience providing case management, crisis intervention, and de-escalation support in a direct service setting.
• Strong communication, conflict resolution, multitasking, and interpersonal skills.
• Fluency in Spanish is highly desired.
• Strong computer skills, including experience with case management databases (e.g., New Org) and familiarity with HMIS reporting preferred.
• Ability to manage multiple priorities, maintain accurate documentation, and meet reporting requirements in a fast-paced environment.
• Ability to exercise sound judgment, work independently, and collaborate effectively with staff, clients, and external service providers.

• Monday-Friday between 7:00 am to 3:30 pm PST.
