Student Health Services Liaison – School-Based Health Center in Ashtabula, Ohio at Ashtabula County Medical Center

The Health Services Liaison collaborates with school nurses, school administration, and clinical teams to coordinate transportation to the school-based health center. The liaison provides day-to-day coordination, scheduling, and logistics pertaining to the transportation process. The liaison also escorts on-campus students to and from appointments in the school-based health center.

• Coordinates transportation for off-campus students to appointments at the school based health center.
• Escorts on-campus students to and from the school-based health center for appointments.
• Assists, as needed, in obtaining, organizing, and communicating patient information such as parental consent, basic demographics, and insurance information.
• Assists, as need, with general functions of the school-based health center which may include appointments, confirmation calls, patient check in / check out.
• Tracking data including, but not limited to, number of students transported, schools from which student was transported, grade levels of students, etc., as determined by the Practice Manager and funding organization.
• High School Diploma required
• Demonstrated organizational and time management skills.
• Ability to multi-task and adapt to changing circumstances.
• Outstanding verbal and written communication skills, required.
• Demonstrated knowledge of computer software.
• Previous experience in education preferred.
