True Health NY is seeking an experienced, dependable and patient-centered Nurse Practitioner to join our growing Brooklyn primary-care practice.

We welcome appropriately certified Family, Adult or Adult-Gerontology Primary Care Nurse Practitioners whose New York certification, training and experience support comprehensive outpatient adult primary care.

The successful candidate will provide preventive, acute and chronic care, help build a stable patient panel and work collaboratively with the site’s primary-care physician and multidisciplinary clinical team.

This is a Brooklyn site-anchored position—not a four-office float assignment. The Nurse Practitioner will have a defined recurring schedule, dedicated clinical support and clear responsibility for documentation, results, follow-up and continuity of care.

Clinical Responsibilities

The Nurse Practitioner will:

  • Provide comprehensive outpatient adult primary care within the clinician’s New York certification, training, experience and demonstrated competency.

  • Conduct new-patient examinations, annual physicals, preventive visits and follow-up visits.

  • Provide Medicare wellness services when qualified, credentialed and assigned.

  • Provide same-day and acute-care appointments when clinically appropriate.

  • Evaluate, diagnose and manage common acute and chronic medical conditions.

  • Manage conditions including hypertension, diabetes, hyperlipidemia, obesity, thyroid disease, asthma, COPD, common gastrointestinal conditions, musculoskeletal complaints and other adult primary-care needs.

  • Perform medication reconciliation and ongoing medication management.

  • Provide evidence-based preventive screening and immunization counseling.

  • Order and interpret laboratory, imaging and diagnostic testing within scope.

  • Review and communicate clinically significant findings promptly.

  • Develop clear treatment, monitoring and follow-up plans.

  • Coordinate referrals and consultation with physicians and specialists.

  • Identify patients requiring physician consultation, emergency evaluation, hospitalization or specialty escalation.

  • Provide appropriate post-hospital and emergency-department follow-up.

  • Provide telehealth services when clinically appropriate and operationally approved.

  • Participate in population-health, chronic-disease and preventive-care initiatives.

  • Support appropriate PCP assignment and patient-panel development in accordance with payer and organizational requirements.

Scope, Consultation and Clinical Escalation

The Nurse Practitioner will:

  • Practice only within the specialty area in which the clinician is certified and registered in New York.

  • Remain professionally responsible for the diagnoses and treatment personally provided.

  • Consult the site physician when a patient’s condition is uncertain, higher acuity, outside the NP’s certified specialty or beyond demonstrated competency.

  • Follow applicable written practice protocols and collaborative-practice requirements.

  • Maintain any collaborative practice agreement required based on the NP’s qualifying experience and New York law.

  • Escalate urgent or unstable conditions promptly.

  • Maintain clear documentation of consultation, referral and escalation.

  • Avoid providing services outside the NP’s training, certification, credentialing or approved clinical privileges.

True Health NY will determine the appropriate practice and collaboration documents based on the selected candidate’s New York certification and qualifying NP practice experience.

Preventive and Chronic-Care Responsibilities

The Nurse Practitioner will help provide consistent care for:

  • Hypertension

  • Diabetes and prediabetes

  • Hyperlipidemia

  • Obesity and weight-related conditions

  • Thyroid disease

  • Asthma and common respiratory conditions

  • Adult immunizations

  • Smoking-cessation counseling

  • Cancer screening

  • Cardiovascular-risk reduction

  • Medication adherence

  • Care-gap outreach

  • Post-hospital follow-up

  • Appropriate specialist referral and follow-through

All care must remain evidence-based, medically appropriate and within the clinician’s certified specialty and demonstrated competency.

Integrated Care Responsibilities

The Nurse Practitioner will collaborate with the clinic’s primary-care physician, OB/GYN clinicians and women’s-health team to:

  • Coordinate primary care for patients receiving gynecologic or prenatal services.

  • Address chronic medical conditions that affect reproductive and women’s health.

  • Support preconception medical optimization within primary-care scope.

  • Coordinate postpartum transition back to ongoing primary care.

  • Promote preventive-care and screening completion.

  • Reduce avoidable duplication of testing and fragmented referrals.

  • Maintain clear boundaries between primary-care and specialty responsibilities.

Documentation and Patient Follow-Through

The Nurse Practitioner will be expected to:

  • Complete encounter documentation accurately and within established practice timeframes.

  • Enter all required orders and charge-support information promptly.

  • Review and act on laboratory, imaging and diagnostic results.

  • Communicate clinically significant findings clearly and promptly.

  • Respond appropriately to patient messages and refill requests.

  • Document referrals, follow-up instructions and care plans clearly.

  • Maintain an organized clinical inbox without a persistent unresolved backlog.

  • Ensure appropriate clinical handoff and cross-coverage when unavailable.

  • Close care loops involving abnormal findings, referrals and follow-up testing.

  • Follow clinical, regulatory, payer and organizational requirements.

Collaboration and Team Responsibilities

The Nurse Practitioner will:

  • Collaborate respectfully with physicians, other advanced-practice clinicians, medical assistants, sonographers and administrative staff.

  • Participate in case review, quality improvement, patient-safety and workflow initiatives.

  • Accept appropriate clinical feedback and implement agreed corrective actions.

  • Help develop efficient workflows that improve access without compromising quality.

  • Support a culture of accountability, responsiveness and patient-centered care.

  • Represent True Health NY professionally in the Brooklyn community.

Patient-volume expectations will be introduced through a structured onboarding ramp and adjusted for patient complexity, new-patient volume, preventive services, procedures and clinical quality.

The position is not based on an inflexible visit count regardless of clinical circumstances.

Required Qualifications
  • Current and unrestricted New York Registered Professional Nurse license and registration

  • Current New York Nurse Practitioner certificate and registration in Family Health, Adult Health or another adult primary-care specialty appropriate to the assigned duties

  • Current applicable national Nurse Practitioner board certification

  • Master’s degree or higher from an accredited Nurse Practitioner program

  • Active New York prescriptive privileges, or clear eligibility to obtain them

  • Active DEA registration, or clear eligibility to obtain one

  • Current NPI

  • Current BLS certification

  • Eligibility for Medicare, Medicaid and commercial-payer credentialing

  • Eligibility for professional malpractice coverage

  • Eligibility to participate in federal and state healthcare programs

  • At least two years of relevant outpatient primary-care Nurse Practitioner experience preferred

  • Strong clinical judgment, communication and documentation skills

  • Ability to work consistently at the Brooklyn location

Preferred Qualifications
  • Three or more years of outpatient primary-care NP experience

  • Family Nurse Practitioner or Adult-Gerontology Primary Care Nurse Practitioner background

  • Experience developing or managing a longitudinal adult patient panel

  • Experience managing Medicare, Medicaid and medically complex adult patients

  • Experience with diabetes, hypertension, obesity and preventive-care programs

  • More than 3,600 hours of qualifying Nurse Practitioner practice experience

  • Experience with eClinicalWorks or a comparable electronic medical record

  • Experience in a community-based or multisite medical practice

  • Strong recent professional references

  • Additional language skills relevant to the Brooklyn patient population

This position is intended for an experienced primary-care Nurse Practitioner. New graduates may be considered for other supervised or structured-development opportunities but are not the primary recruiting target for this site-anchor role.

Compensation

True Health NY expects to offer an annual base salary of $145,000–$170,000 for this position.

The final base salary will depend on factors including:

  • New York certification specialty

  • National board certification

  • Years of Nurse Practitioner experience

  • Relevant outpatient primary-care experience

  • Scope and demonstrated competency

  • Qualifying NP practice hours

  • Schedule

  • Credentialing readiness

  • Professional references

  • Internal equity

  • Other lawful considerations

The selected clinician may also be eligible to participate in a written quality and productivity incentive plan after completing the initial onboarding and performance-ramp period.

Incentive compensation is separate from the stated base-salary range and will be explained before an offer is accepted.

A sign-on bonus may be available for a qualified candidate and, when provided, will be subject to a written service and repayment agreement.
