Case Manager (LPN Requirement)
Field of Dreams Assisted Living is seeking a motivated, compassionate, and relationship-driven Licensed Practical Nurse (LPN) to serve as our Case Manager supporting resident admissions, care coordination, and community outreach within our Assisted Living and Memory Care communities.
This role is critical in supporting individuals living with dementia and cognitive impairment, as well as their families, by guiding them through the admissions process and ensuring residents receive high-quality, person-centered care. The ideal candidate understands the complexity and importance of memory care, recognizes the emotional needs of residents and families, and can effectively promote Field of Dreams' services while serving as a clinical resource and advocate for residents.
Key Responsibilities
• Serve as a primary point of contact for prospective residents, families, hospitals, physicians, and referral sources.
• Coordinate admissions and transitions of care for Assisted Living and Memory Care residents in collaboration with clinical and administrative teams.
• Evaluate resident needs to ensure appropriate services, care plans, and resources are in place.
• Monitor resident status and communicate changes, concerns, and recommendations to appropriate healthcare providers and family members.
• Maintain communication with residents and families to support retention.
• Develop and strengthen referral relationships with hospitals, providers, rehabilitation centers, home health agencies, and community partners.
• Participate in marketing and outreach efforts, including tours, follow-up communications, professional networking, and community events.
• Accurately document case management activities, level of care coordination efforts, and resident information in accordance with regulatory requirements.
• Assist with care plan development and ensure services remain aligned with resident needs.
Qualifications
• Current New York State Licensed Practical Nurse (LPN) license required.
• Experience in assisted living, memory care, long-term care, home care, senior services, or related healthcare setting preferred.
• Previous experience in case management, care coordination, admissions, marketing, sales, or resident services preferred.
• Knowledge of Alzheimer's disease, dementia care, and person-centered care practices.
• Strong interpersonal, communication, customer service, and organizational skills.
• Ability to build trust and meaningful relationships with residents, families, referral sources, and healthcare professionals.
• Ability to multitask, prioritize, and work independently in a fast-paced environment.
• Proficiency with electronic health records and basic office software.
