C-Care Health Services is urgently hiring Personal Support Workers (PSWs) who are available and interested in working in both home care and facility settings across Toronto.

Position Summary

As a Personal Support Worker with C-Care, you may be assigned to provide care in private homes, healthcare facilities, or escort support appointments, depending on client needs and your availability.

This role is ideal for PSWs who are reliable, flexible, professional, and comfortable supporting clients with varying levels of care needs.

Key ResponsibilitiesHome Care Setting
  • Assist clients with daily personal care, including bathing, grooming, toileting, dressing, and hygiene support
  • Support clients with mobility, ambulation, positioning, and transfers
  • Safely assist with transfers between beds, chairs, wheelchairs, and other mobility supports
  • Help prevent falls and injuries by following safe care practices
  • Provide meal support, including assistance with eating, meal reminders, and monitoring dietary needs or restrictions
  • Provide companionship, emotional support, conversation, and encouragement
  • Support clients with light home support tasks related to their care needs
  • Observe and report changes in client health, behaviour, mood, mobility, or overall well-being
  • Follow the client’s care plan and communicate any concerns to the appropriate supervisor
Facility Setting
  • Provide care in accordance with facility, hospital, or long-term care policies and procedures
  • Follow assigned care plans and directions from facility staff or healthcare professionals
  • Assist residents or patients with personal care, mobility, feeding, and comfort needs
  • Support nursing and healthcare teams as required within the PSW scope of practice
  • Maintain professionalism, confidentiality, and respectful communication in all facility environments
  • Accurately report client or resident concerns, changes, or incidents as required
Medical Escort Duties
  • Accompany clients to and from medical appointments, procedures, errands, or approved destinations
  • Coordinate safe transportation and help ensure clients arrive on time
  • Assist with walking, standing, transfers, and mobility during transport
  • Remain with the client during the escort service as required
  • Monitor for signs of discomfort, weakness, confusion, or post-procedure concerns
  • Provide appropriate aftercare support following appointments, procedures, or discharge
  • Communicate updates or concerns to the office and/or family contact as required
Minimum Qualifications
  • Personal Support Worker certificate/diploma from an accredited institution
  • NACC certification required if your PSW diploma is from a private career college
  • Valid First Aid and CPR certification
  • Valid Mask Fit certification
  • Strong English communication skills, both written and verbal
  • Ability to follow directions from healthcare professionals, supervisors, and care plans
  • Ability to work independently and professionally in private homes and facility settings
  • Reliable, punctual, compassionate, and client-focused
  • Experience supporting clients with Dementia, Alzheimer’s disease, Parkinson’s disease, ALS, or complex care needs is highly preferred
Additional Requirements

Successful candidates may be required to provide:

  • Vulnerable Sector Check / Police Clearance
  • Updated immunization records
  • COVID-19 vaccination records, where required by client or facility policy
  • Proof of eligibility to work in Canada
  • Professional references
  • Relevant training certificates, if applicable
Preferred Skills and Experience
  • Experience working in home care, hospitals, retirement homes, or long-term care facilities
  • Comfort working with seniors, adults with disabilities, and clients with cognitive or physical challenges
  • Ability to manage changing client needs with patience and professionalism
  • Strong observation and reporting skills
  • Ability to maintain client dignity, privacy, and independence
  • Experience with transfers, mobility aids, dementia care, and post-operative support

Work Location
