Key Responsibilities:

• Deliver comprehensive primary care services, including preventive screenings, diagnosis, and treatment.
• Integrate SDOH screenings and interventions into patient assessments and care plans.
• Participate in continuous quality improvement initiatives to meet and exceed PCMH quality metrics.

What You’ll Bring

• Active NP license and DEA certification for Louisiana.
• Experience in primary care, acute care, or urgent care preferred.
• Strong critical thinking, communication, and independent decision-making skills.
