Job Description

Position Summary:

The Outpatient Nephrology Advanced Practice Provider (APP), including Physician Assistants (PA) or Nurse Practitioners (NP), provides comprehensive, patient-centered care for individuals with acute and chronic kidney disease in an outpatient clinic setting. Working in collaboration with nephrologists, the APP manages ongoing kidney disease, hypertension, and related metabolic conditions while emphasizing disease prevention, patient education, and long-term care coordination.

Responsibilities and Duties:

  • Conduct comprehensive outpatient evaluations, including history, physical exams, and review of laboratory and diagnostic studies.
  • Diagnose and manage chronic kidney disease (CKD), hypertension, electrolyte disorders, proteinuria, and other renal-related conditions.
  • Monitor disease progression and adjust treatment plans in collaboration with supervising nephrologists.
  • Provide routine follow-up care for stable and complex nephrology patients.
  • Manage anemia of chronic kidney disease, mineral and bone disorders, and other CKD-related complications.
  • Interpret lab results including renal function panels, urinalysis, and relevant imaging studies.
  • Prescribe and manage medications, including antihypertensives, phosphate binders, diuretics, and other nephrology-related therapies within scope of practice.
  • Provide extensive patient education on CKD progression, medication adherence, dietary modifications (e.g., sodium, potassium, phosphorus restriction), and lifestyle changes.
  • Coordinate care with dialysis centers, vascular access teams, primary care providers, and specialty services as needed.
  • Monitor and manage patients transitioning to dialysis or conservative kidney care pathways.
  • Ensure accurate, timely documentation in the electronic medical record (EMR).
  • Participate in quality improvement initiatives and adherence to evidence-based nephrology guidelines.

Qualifications:

  • Graduate of an accredited Physician Assistant or Nurse Practitioner program.
  • Current national certification (NCCPA, ANCC, or AANP) preferred.
  • Active and unrestricted state licensure as an APP.
  • Prior experience in nephrology, internal medicine, or primary care preferred; new graduates welcome.
  • Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications are preferred.
  • Proficiency with electronic medical records (EMR).
  • Strong clinical judgment, communication, and organizational skills.

Competitive Salary based on experience: $110,000 to $145,000 per year DOE
