Are you an LCSW passionate about supporting women and families during pregnancy and postpartum? Join Bridge Prenatal as a Resource Companion and make a meaningful difference in the lives of mothers and families.

This position is primarily remote, with occasional in-person visits with patients in the Riverside, California area.

• Conduct psychosocial, behavioral health, and social determinants of health assessments.

• Support women throughout pregnancy and the postpartum period.

• Provide education, counseling, and compassionate psychosocial support.

• Connect patients with community resources and services.

• Develop individualized plans of care.

• Collaborate with physicians, nurses, lactation professionals, and other care team members.

• Complete timely and accurate clinical documentation.

• Identify and appropriately escalate urgent behavioral health needs.

• Active California LCSW license – REQUIRED

• 3+ years of mental health counseling or social work experience preferred.

• Perinatal, maternal health, women's health, or healthcare experience is a plus.

• Strong assessment, communication, documentation, and care coordination skills.

• Compassionate and culturally responsive approach.

• Ability to work independently in a remote environment.

• Reliable transportation for occasional in-person visits.

• Bilingual skills are a plus.
