Job Description
Experience and Skills:
• Strong understanding of reentry services, care transitions, and evidence-based practices for under-resourced populations.
• Proven ability to develop and sustain community partnerships.
• Experience in program implementation, data management, and reporting.
• Knowledge of community health services, housing resources, and behavioral health interventions.
• Compassionate, strengths-based approach to working with vulnerable individuals.
• Excellent organizational, communication, and problem-solving skills.

- knowledge of electronic medical records.
- knowledge of community and community resources.
- ability to pass background check.
- valid California State Driver’s License.
- Flexibility to work overtime, holidays and weekends when necessary
- Local travel required.
- Successful completion and acceptable results of post offer physical examination, drug screening, and criminal background check
- Requirements for vaccinations and preventative testing: Influenza, MMR, VZV, Tdap, Hep B and TB
- License Registration with licensing board
