DISCHARGE PLANNER POSITION SUMMARY

The Discharge Planner remains focused on connectivity, preparing the inmate-patient for transitioning into the community. This role involves a great deal of information gathering, compiling discharge summaries from various sources, providing chronic care patients with a summary of their healthcare treatment while in the custody of the Department of Correction. This service is provided for those patients who are scheduled for release, as well as those who are suddenly released (i.e., pre-sentenced detainees).


RESPONSIBILITIES:

  • Identifies chronic care patients and reviews medical, mental health, and substance abuse treatment needs
  • Develops written individualized Discharge Summary, including relevant diagnoses, test results, findings, and treatment while in custody of the Department of Correction
  • Reviews the risks and needs of each chronic care patient and identifies community-based providers capable of providing ongoing treatment
  • Reviews health care records of all chronic care patients in order to develop Discharge Summaries
  • Consults with medical, mental health, and substance abuse treatment providers when appropriate
  • Consults with Case Managers to ensure patient has the means for connecting with the community-based treatment providers
  • In consultation with other treatment staff, maintains database of community-based treatment providers for various specialties· Respects the dignity and confidentiality of patients
  • Attends mandatory staff meetings and trainings
  • Performs other duties as assigned

  • Clinical Knowledge: Possesses understanding of assessment and intervention skills, specifically evidence-based practices. Implementation of these skills is done in a professional manner.
  • Critical Reasoning: Excellent observation skills and the ability to combine pieces of information to form general rules or conclusions, as well as extracting themes or patterns from seemingly unrelated pieces of information
  • Communication: Ability to convey complicated clinical and professional information to various audiences, in both written and oral form. Understanding the context in which information exists, and the ability to temper opinions diplomatically, all in an effort to model mature, positive, and appropriate behavior. Written communication is edited for spelling and grammar, and presents numerical and scientific data appropriately in clear, concise, and accurate manner.
  • Adaptability: Quickly adapts to changes in the work environment, manages competing interests, and demonstrates the ability to deal with frequent changes, delays, or unexpected events
  • Stress Tolerance: Recognizes crisis situations and reacts to such in a calm, mature, professional manner. Accepts criticism and feedback, and is able to deal calmly and effectively with high stress situations

DISCHARGE PLANNER REQUIREMENTS:

  • Minimum one year of clinical experience (Preferred)
  • Must have and maintain current licensure as a Licensed Practical Nurse or Registered Nurse in Delaware
  • Pre-employment Tuberculin Skin Test and CPR Certification
  • Must maintain Full Security Clearance, approved by the Delaware Department of Correction
  • Satisfactory background investigation report

HEALTH COACH CASE & MANAGER POSITION SUMMARY


The Health Coach & Case Manager oversees various components of wellness education and connectivity, linking the patient with information, services, and supports during and after incarceration. The Health Coach & Case Manager provides wellness information for patients with chronic care concerns, such as diabetes, hypertension, obesity, and hepatitis, using materials developed by the Director of Education and Case Management. Additionally, an awareness of social supports and services within the community, such as connecting patients with Medicaid and insurance, is critical to the position.


RESPONSIBILITIES:

  • Identifies high-risk populations and provides wellness education, encouraging patients to lead healthier lives
  • Implements curricula developed by Director of Education and Case Management, which focuses on providing patients with information regarding chronic care and healthy living
  • Assesses infrastructure issues pertaining to the individual patient and prioritizes patients with the highest risks and the greatest needs
  • Provides education on social service infrastructure components, such as accessing Medicaid, Healthcare.gov, and other components relating to the Affordable Care Act
  • Conducts post-release follow-up with patients to assess outcomes regarding connectivity, and to continuously evaluate the effectiveness of education and case management
  • Provides data to Statewide Performance Improvement team for analysis
  • Consults with Discharge Planners as appropriate
  • Respects the dignity and confidentiality of patients
  • Attends mandatory staff meetings and trainings
  • Performs other duties as assigned
