LCSW-C Position Summary

The LCSW-C for Palliative Care provides specialized psychosocial support to hospitalized patients and their families receiving Palliative Care. This role is an integral part of the Palliative Care Team, offering emotional support, resource referrals, and guidance during complex medical decisions. This position documents Social Work assessment, family meetings, interactions, family dynamics, and other pertinent information in the medical record. The Social Worker assists patients with completing advance directives and/or aides in obtaining previously completed advance directives. Also assists with triage of new consults as needed.

Education

• Graduate of a Master’s degree program in social work from a college or university approved by the Council of Social Work.

Required License and/or Certification

• Maryland State Social Work license, at the Licensed Certified Social Worker – Clinical (LCSW-C) level.

Experience

• Knowledge of community resources and agencies is preferred.
• Proficient computer skills - Epic EHR experience is preferred.
• Knowledge of HIPAA and confidentiality, and patient rights and laws.
• 1 year of experience in a healthcare setting and/or experience with crisis intervention and supporting patients with psychosocial barriers to health is preferred.

LCSW-C Work Schedule

• Primarily day shift Monday through Friday.
• Evening and weekend work as needed.
