Social Worker (LBSW) Position Summary

The Social Worker-LBSW is responsible for the clinical assessment, case management, referral to next level of care, patient advocacy, and patient education related to outpatient resources. The Social Worker attends treatment team regularly to advocate for the patient related to the discharge plan. The Social Worker coordinates and collaborates with other departments and the community to achieve best treatment outcomes for patients.

Social Worker (LBSW) Position Requirements

Education

• Bachelor’s degree in Social Work required.

Required License and/or Certification:

• Licensure as a Social Worker in the State of MD required.

Experience

• Minimum of 3-5 years previous experience in social work or related capacity preferably in medical/health setting.

Salary range: $28.26 - $43.81 Commensurate with experience
