Healthcare Outreach Manager in Cincinnati, Ohio at DeCoach Rehabilition Center

Healthcare Outreach Manager is responsible for developing, managing, and growing DeCoach Recovery Centre’s healthcare referral network. This position builds strong relationships with hospitals, emergency departments, discharge planners, social workers, behavioral health teams, medical providers, and community healthcare partners to create clear pathways into treatment.

This role also provides leadership and oversight to a team of Healthcare Liaisons, ensuring consistent outreach strategy, accountability, professional relationship-building, and timely follow-up with healthcare referral sources. The Healthcare Outreach Manager plays a key role in helping individuals access detox, residential treatment, PHP, IOP, outpatient care, medication-assisted treatment, mental health services, court services, transportation, and recovery support.
