About This Position

POSITION SUMMARY

The Rapid Rehousing Stability Case Manager’s (RRSCM) primary objective is to empower the client with the resources and support needed for self-sufficiency, address immediate barriers and achieve long-term permanent housing. This encompasses overall delivery of services and implementation of individualized case plans for families and/or individuals experiencing homelessness and who are at high risk of severe illness due to COVID-19. The position will be responsible for providing case management services specifically for housing placement services, temporary financial assistance, conduct harm reduction activities, job placement opportunities and other support services.

PRIMARY DUTIES AND ESSENTIAL FUNCTIONS

  • Provide a range of client-centered services to support persons who are housing through:
    • Maintain and conduct a case load of twenty (20) assessment of all clients requesting RRH Assistance;
    • In collaboration with the client, develop a Client-driven service plan which outlines goals so clients receiving RRH assistance that will achieve long term housing stability; and
    • Use a Strength-Based approach to empower clients and understand the treatment term of Trauma-Informed Care.
  • Complete data entry for assistance in the Homeless Management Information System (HMIS) database.
  • Responsible for assisting walk-in clients with basic needs and linking walk-in clients to appropriate services at CPLC NEVADA or in in the community.
  • Assist with determining types of housing assistance clients will receive in conjunction with the CPLC NEVADA Housing Counseling Team (as applicable and needed).
  • Actively participate in weekly/biweekly program & staff meetings.
  • Conduct home visits to assess and support client and/or identify unmet needs and institutional visits as applicable.
  • Attend and participate in treatment team meetings with medical providers and network with other agency and community service providers to secure appropriate, quality services for program clients.
  • Ensure housing participants understand their tenancy/participation agreements, their rights and obligations as participants, and their CPLC NEVADA client rights and responsibilities.
  • Comply with CPLC NEVADA employment and quality assurance policies and procedures.
  • Work closely with LRRHCM and additional case managers to ensure program compliance and deliverables.
  • Build relationships with other local Rapid Rehousing service providers to develop referral sources and share best practices.
  • Assist with other outside counseling and teach or assist with financial education workshops and crisis budgeting classes as needed.
  • Ad hoc duties as assigned.

MINIMUM QUALIFICATIONS AND COMPETENCIES

Education /Background:

  • Bachelor’s degree in social work or related field and one year's experience working with the mentally ill, intellectual disabilities, and/or substance abuse.

Other Requirements:

  • Working knowledge of housing practices including NV Landlord Tenant Act, Fair Housing, Housing Quality Standards (HQS), and Property Management.
  • Marketing and salesmanship skills, ability to meet a demanding workload and multi-task throughout work schedule.
  • Ability to travel statewide and work a flexible schedule if needed.
  • Must pass background screening and TB test prior to start.
  • Continuous and ongoing education.
  • Excellent written and verbal skills in the English language. Bilingual English-Spanish preferred
